We are searching for a Registered Nurse Case Manager to join our growing care management team! In this role, you will develop care initiatives to assess and coordinate chronic disease care management for patients. You will act as a patient advocate and ensure every patient receives the quality care they deserve.

Responsibilities:

• Collaborate with Primary Care Physicians and all members of a patient’s healthcare team.
• Assessing patients, gathering information, reviewing diagnoses, and analyzing medical test results.
• Developing care plans that meet the needs of patients and revising plans based on patient’s progress toward goals.
• Educating patients and families about care plans and their options.
• Updating patient’s medical records.
• Act as a resource and mentor to new employees.
• Assess the ongoing educational needs of the departments.

Qualifications:

• Graduate from an accredited nursing program.
• Active RN License.

Schedule:

• Full-time, 32-36 hours/week
• Schedule: 4 days/week, 8a-530p or 8a-5p
• NO WEEKENDS!
