We are searching for a Collaborative Care Registered Nurse to join our Care Management team. In this role, you will work closely with a group of patients to assist them in the self-management of their chronic illnesses, including dialysis, COPD, asthma, hypertension, etc.

Responsibilities:

• Perform as a member of the healthcare team to apply the nursing process when making nursing judgments and decisions specific to assessment, planning, implementation/delivery, and evaluation of patient care, and documenting relevant data and care processes.
• Facilitate communication between the patients and their Primary Care Providers.
• Assist with Advance Directives planning, Annual Wellness Visits, chronic disease action plans, and preventative care utilizing assessment skills both in person and telephonically.
• Serve as a resource for other team members.

Qualifications:

• Graduate from an accredited nursing program.
• Active RN License.
• 2 years of nursing experience is preferred.

Schedule:

• Full-time, permanent position
• 4 days, 36 hours/week, 8a-530p
• NO WEEKENDS!
