Cheshire County Government is Hiring a TrECC Care Coordinator Near Claremont, NH

General Summary:

Transitional Enhanced Care Coordination (TrECC) is a family-driven process for helping youth with significant mental and behavioral health challenges, who may be undergoing psychiatric hospitalization or an episode of treatment in a residential setting. The TrECC Coordinator will support these youth and their families to successfully transition from one setting to the next, ensuring that the youth is in the least restrictive environment and has access to the appropriate supports in their targeted community.

Essential Job Functions

· Maintains a caseload of 8-10 youth and families.

· Advocates for youth and family and ensures their voices are heard.

· Ensures treatment plans are relevant and transition plans are realistic.

· Supports episodes of residential treatment and transition planning.

· Facilitates TrECC team meetings.

· Facilitates family meetings.

· Works in conjunction with youth, family and existing teams and the residential treatment program by:

o Supporting youth and families in the referral process including tracking of referrals to episodes of residential treatment.

o Attending treatment team meetings.

o Coordinating and making referrals to supportive community services for up to six months following transition.

o Assisting families with applying for services or Medicaid.

o Conducting ongoing assessments by utilizing the CANS & Preparedness Tool.

· Engages with coaching in accordance with TrECC standards

· Enters all appropriate case specific information into the data system.

· Must be independent, organized, flexible, reliable, creative, and work collaboratively as part of a team.

· Adheres to ethical, Medicaid required reporting, and confidentiality standards as required by state laws and TrECC standards

· Will work towards TrECC certification within the first two years of employment.

Qualifications/Education/Training

· A minimum of a Bachelor’s degree from a recognized college or university in related field.

· A minimum of two years’ experience, in health care, social services, behavioral health care, social work, human services, or education, and preferably with care coordination experience.

· A high school diploma or equivalency; and a) was a participant in, or is a direct caregiver, or was a direct caregiver of an individual who received services from the public child and family-serving system; and b) meets the training and certification requirements for the care coordinator.

· Must maintain valid drivers license, current NH vehicle registration and inspection as well as insurance with a minimum of $50,000/$100,000 for bodily injury & property damage liability.

Job Type: Full-time

Pay: From $27.21 per hour

Expected hours: 40 per week
