As a WCBH Care Coordinator, you'll work closely with our Clinical Team to provide care coordination and case management services for a caseload of clients receiving community based and in office skills-based therapy. The primary role of the Care Coordinator will be to assess needs, coordinate care and connect individuals with community resources. The Care Coordinator will also support clinical processes associated with mental health treatment when applicable.
