Position Overview: CHEMED Health is seeking a highly skilled Licensed Clinical Social Worker (LCSW) to join our dynamic team as an Intake Specialist. The LCSW will play a pivotal role in the initial assessment and triage process, ensuring that patients are efficiently and effectively connected with the appropriate LCSW for ongoing care.
Key Responsibilities:
Conduct comprehensive psychosocial assessments of individuals seeking services at CHEMED Health.
Utilize clinical expertise to evaluate patients' needs, including mental health concerns, social determinants of health, and relevant support systems.
Collaborate with interdisciplinary teams to gather pertinent information and develop individualized care plans.
Provide brief interventions and crisis management as needed during the intake process.
Serve as the primary point of contact for patients during the intake phase, offering empathy, support, and guidance.
Utilize electronic health records (EHR) systems to accurately document assessment findings and treatment recommendations.
Work closely with other LCSWs and mental health professionals to facilitate seamless transitions and referrals for ongoing care.
Stay informed about community resources and referral networks to effectively connect patients with additional support services.
Qualifications:
Master's degree in Social Work from an accredited institution.
Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
Minimum of 2 years of clinical experience in a healthcare or mental health setting, with a focus on intake assessments preferred.
Strong understanding of diagnostic criteria and evidence-based treatment modalities for mental health disorders.
Excellent communication and interpersonal skills, with the ability to establish rapport and build trust with diverse populations.
Proficiency in conducting psychosocial assessments and formulating treatment plans.
Familiarity with electronic health records (EHR) systems and other technology platforms.
Commitment to cultural competence, sensitivity, and trauma-informed care principles.
Ability to work collaboratively within a multidisciplinary team environment
