Position Overview:CHEMED Health is seeking a highly skilled Licensed Clinical Social Worker (LCSW) to join our dynamic team as an Intake Specialist. The LCSW will play a pivotal role in the initial assessment and triage process, ensuring that patients are efficiently and effectively connected with the appropriate LCSW for ongoing care.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments of individuals seeking services at CHEMED Health.
  • Utilize clinical expertise to evaluate patients' needs, including mental health concerns, social determinants of health, and relevant support systems.
  • Collaborate with interdisciplinary teams to gather pertinent information and develop individualized care plans.
  • Provide brief interventions and crisis management as needed during the intake process.
  • Serve as the primary point of contact for patients during the intake phase, offering empathy, support, and guidance.
  • Utilize electronic health records (EHR) systems to accurately document assessment findings and treatment recommendations.
  • Work closely with other LCSWs and mental health professionals to facilitate seamless transitions and referrals for ongoing care.
  • Stay informed about community resources and referral networks to effectively connect patients with additional support services.

Qualifications:

  • Master's degree in Social Work from an accredited institution.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
  • Minimum of 2 years of clinical experience in a healthcare or mental health setting, with a focus on intake assessments preferred.
  • Strong understanding of diagnostic criteria and evidence-based treatment modalities for mental health disorders.
  • Excellent communication and interpersonal skills, with the ability to establish rapport and build trust with diverse populations.
  • Proficiency in conducting psychosocial assessments and formulating treatment plans.
  • Familiarity with electronic health records (EHR) systems and other technology platforms.
  • Commitment to cultural competence, sensitivity, and trauma-informed care principles.
  • Ability to work collaboratively within a multidisciplinary team environment
