Charlotte Community Health Clinic is Hiring a Nurse Practitioner (NP) Near Charlotte, NC

Job Description

Charlotte Community Health Clinic offers high-quality medical, dental and behavioral health services for children and adults. We are currently seeking a Family Nurse Practitioner to join our team. Responsibilities include providing high quality medical care, patient education, strict adherence to HIPPA guidelines, enjoy working in team environment, and involvement in Quality Improvement activities. This is a full-time position and will report to the Chief Medical Officer.

Duties:

  • Provide medical services to patients, inclusive of all functions necessary to provide quality medical care
  • Develop treatment plans for acute and chronic diseases
  • Educate and guide patients on disease prevention, disease management and healthy lifestyle habits
  • Provide monitoring and continuity of care between patient visits
  • Contribute to teamwork and communications
    • Work within a clinical care team to achieve patient and team goals, participate in regular staff meetings, and collaborate with other disciplines to achieve desired outcomes
    • Work within the provider team, including participating in regular staff meetings and collaborating with other providers to achieve desired outcomes for the clinic’s patient population as a whole
    • Collaborate with physician clinical supervisor for chart review and continued education
  • Provide administrative, staff development and student supervisory responsibilities as needed
  • Participates in Quality Improvement activities

  • Collaborative community advocate for community health for a vulnerable patient population, growing knowledge of the community resource network for low-income and other vulnerable patients and making referrals as needed for patient care and with consideration of limited patient resources
  • Continuous self-educator on providing quality care and improving professional skills
  • Team orientation
  • Openness to evolution and change in how we deliver care, as Charlotte Community Health Clinic continuously evolves to advance the wellbeing of our patients and community

Qualifications:

  • MSN with FNP concentration; post master’s certificate with FNP concentration; or DNP with FNP concentration required
  • Current NP license in North Carolina, registered with NC State Board, current DEA registry and BLS.
  • Minimum of three years’ experience in community based programs, including low-income, uninsured patients
  • Bilingual in Spanish and English preferred
  • National certification by AANPCP or ANCC as a Family Nurse Practitioner.
  • Experience with EPIC EMR preferred
