Description

• Determine client eligibility and complete applications.
• Assist clients in identifying priorities and setting a specific ROMA goal for self-sufficiency.
• Develop and monitor action plan to assist clients towards their goals of self-sufficiency.
• Compiles and maintains record files for accuracy.
• Refers clients to community resources and other organizations for services not rendered by Palmetto Community Action Partnership.
• Accurately enters and maintains electronic documentation through computerized record keeping, along with manual documentation as required for operational efficiency.
• Accurately documents and reports clients’ outcomes according to the three national ROMA goals.
• Maintains and submits bi-weekly case notes and action plan.
• Promptly attends staff meetings, supports and participate in agency functions in addition to community meetings.
• Disseminates information to parents, students and the community regarding agency programs.
• Assist with the coordination and marketing of program workshop.
• Assist students who required special accommodations.
• Other duties as assigned by the lead case manager/s or E&E Director.

Qualifications

Minimum 2-year degree in Human Services, Social Services or related field desired or a combination of education and /or experience

• Must have the ability to relate to our target population.
• Must be able to work in a fast-paced environment.
• Ability to work with a team as well as work independently.
• Must possess effective verbal and written communication skills.
• Must be computer literate with data entry experience and a strong attention to detail.
• Must have dependable transportation.
• Basic knowledge of MS and Teams.
