Position Overview: The Continuing Care Coordinator is a crucial member of our addiction treatment facility's team, responsible for supporting patients as they transition from inpatient treatment at Charles River Recovery to the next phase of their recovery journey. The primary objective of this role is to work with Case Management to support patients in either creating a solid aftercare plan, or to assist in keeping them on track with their existing one. The Continuing Care Coordinator will facilitate and coordinate ongoing care and support services for clients, ensuring their continued success in maintaining sobriety and achieving a fulfilling, healthy life in recovery. In addition to patient-focused responsibilities, this role includes supporting business-to-business (B2B) relationships with external organizations to enhance the continuum of care and resources available to our patients.
Specific Responsibilities:
- Continuing Care Planning: Collaborate with the treatment team, patients, and referents to create a personalized continuing care plan that addresses the unique challenges and goals of each individual.
- Resource Referral: Identify and connect clients with external resources and support services, PHP or IOP programs, etc., to meet their specific needs.
- Monitoring and Follow-Up: Regularly check in with clients to track their progress, address any challenges, and provide ongoing support. Offer guidance, encouragement, and motivation to help them stay on track with their recovery goals.
- Crisis Intervention: Be available to provide crisis intervention services, offering immediate assistance to patients when they are at risk of leaving against advice or facing other issues that may interfere with their continuing care. Communicate with referents and other members of the outreach team to leverage available resources.
- Education and Advocacy: Educate clients about the importance of ongoing recovery efforts and advocate on their behalf to access necessary services and resources.
- Documentation: Maintain updated census/record of continuing care efforts and facilitate effective communication with the treatment team.
- Collaboration: Work closely with therapists, counselors, and medical professionals within the facility to ensure a seamless transition from inpatient or outpatient care to continuing care.

Education and Experience:
- Bachelor's degree in social work, psychology, counseling, or a related field preferred.
- Knowledge of addiction and recovery principles, relapse prevention strategies, and community resources.
- Excellent communication, interpersonal, and organizational skills.
- Compassionate and nonjudgmental approach to working with patients.
- Ability to work independently and as part of a multidisciplinary team.
- Strong problem-solving and crisis management skills.
- Familiarity with electronic health records (EHR) and documentation requirements.

Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools, or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus.
Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
