Charles River Recovery LLC is Hiring a Case Manager Near Weston, MA

Case Manager

Charles River Recovery is growing, recently added 30 new beds in 2023 patient census, as we provide the highest quality substance use treatment services, directed by evolving physical, emotional, and spiritual needs of our patients, delivered in an environment that fosters well-being and compassionate care, driven by the continuous pursuit of improvement.

The Case Manager will provide case management services and work with the clinical staff to coordinate care and assist with treatment plans for clients.

We are welcoming walk-in interviews Monday - Friday 9a-4p for your interviewing convenience!

Positions include

  • Full-time 40 hours per week
  • Part-time staff - 8 or 16 hours per week,
  • Every other weekend rotation required

Specific Responsibilities:
Participate in the assessments and developments of the service plan.
Implements services to patients in accordance with their individualized service plan (ISP).
Collaborates with the patient and patient’s family or significant others to implement aneffective service plan, explaining the available clinical options to the team, including theadvantages and disadvantages of each option.
Communicates and documents patient’s progress toward their recovery.
Maintains the patient’s comprehensive clinical record, including documentation ofactivities performed as part of the service delivery process (e.g., assessments, provisionof services, coordination of care, discharge planning).
Works collaboratively with the clinical team to engage, educate, communicate, andcoordinate care with patient, their family, behavioral and general medical and dentalhealth care providers, community resources and others to ensure that all servicesprescribed in the individualized service plan (ISP) are implemented. Provides assistance in maintaining, monitoring and modifying covered behavioral healthservices.
Seeks out necessary resources other than covered services to meet basic needs.
Ensures all appropriate referrals for identified services on the service plan are made andcoordinated with service providers via contracted network providers or communityresources; Ensures all covered services identified on the service plan are evaluated andupdated monthly.

Minimum Qualifications
High School Diploma and minimum of one (1) year of professional experience in BehavioralHealth or Psychiatric is preferred
Bachelor’s Degree in a social work field is preferred
Knowledge of health care, detoxification process, addiction, co-occurring disorders, DSMCriteria, and terminology.
Ability to read and interpret written information; write clearly and informatively; edits workfor spelling and grammar. Ability to speak clearly and persuasively in positive or negativesituations; listens and gets clarification; responds well to questions; demonstrates grouppresentation skills; and participates in meetings.
Working knowledge of Microsoft Word, Excel, and Outlook.

Collaboration: Clinical leaders must be able to work in collaboration with other health professionals and leaders from other disciplines.

Work Environment: This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.

Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus.

Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
