As a Navigation Team Lead, you will play a crucial role in advancing our mission of ending homelessness. You will collaborate with community stakeholders, leverage resources, and implement innovative strategies to prevent homelessness and support those in need.

Job Description:

Job Title: SSVF Navigation Team Lead

Department: Veteran Services

Employee Type: Full time, Salary

Exemption Status: Exempt

Salary Range: $45,000 – $51,000

Reports to: Central Inflow Veteran Services Manager

Position Summary:

This position will entail management responsibilities for the Outreach and Navigation Teams working with clients of the Supportive Services for Veteran Families Program and Changing Homelessness.

Position Responsibilities:

  • Program Compliance: Ensures all aspects of Outreach and Navigation Teams comply with VA, HUD, and HMIS standards and requirements
  • Program Reporting: Collects and analyzes data for the team, reporting out to other team leads and senior staff
  • Outreach Planning: Develops a comprehensive quarterly plan to provide consistent, collaborative outreach to the service coverage area to identify prospective Veteran households and form relationships with community partners
  • Data: Ensure that Outreach and Navigation Teams methods are data-driven and in line with SSVF and Continuum of Care goals
  • Outcomes: Work with all teams to ensure outcome measurements are regularly monitored and shared with SSVF and Coalition partners
  • Organize workflow and ensure that staff understand their duties or delegated tasks
  • Assist in the recruitment, onboarding, and training of new staff
  • Monitor staff performance and provide constructive feedback and coaching
  • Prepare and perform 45-90 day reviews and annual performance appraisals
  • Follow agency coaching and performance counseling guidelines should disciplinary actions be needed
  • Communicate information from upper management to employees and vice versa
  • Ensure adherence to legal and company policies and procedures
  • Maintain confidentiality of all staff, agency, and client information
  • Maintain a safe and healthy work environment by establishing and enforcing organization standards; adhering to legal regulations.
  • Provides quality service by enforcing quality and customer service standards.
  • Comply with and promote compliance with the SSVF Program Guide and agency policies and procedures
  • Host and/or participate in weekly Veteran By Name List meetings
  • Host case conferencing and perform reviews of client files to determine if appropriate for exit from the program
  • Schedule “One on One” meetings with Outreach and Navigation Teams to provide individual coaching and development
  • Act as lead for CARF surveying, SSVF monitoring, and all other audits of the program
  • Collaborate with VA, HUD/VASH, Coordinate Entry, and community partners to effectively coordinate services
  • Other duties as assigned

Minimum Position Requirements:

  • Bachelor’s degree or equivalent experience with a minimum of 4 years of social service experience
  • Previous outreach or case management experience preferred
  • Veteran status preferred
  • Experience with homelessness preferred
  • Successfully pass Level 2 background check and drug screening
  • Must have a valid driver’s license and clean 3-year driving history
  • Must have required vehicle insurance within 30 days of onboarding

Knowledge, Skills, and Abilities Required:

  • Strong interpersonal and communication skills
  • Skilled in the use of personal computers and related software applications
  • Skilled in organizing resources and establishing priorities
  • Working knowledge of community resources
  • Ability to provide technical advice and information to faculty and staff in the area of expertise
  • Knowledge of federal, state, and/or community funding sources and mechanisms
  • Ability to communicate effectively, both orally and in writing
  • Knowledge of grant funding policies and procedures and applicable local, state, federal, and CHI regulations
  • Strong public service orientation to work well with staff and other stakeholders
  • Ability to foster a cooperative work environment
  • Strong computer skills
  • Ability to work with a diverse team in a fast-paced environment
  • Enthusiasm and the ability to thrive in an atmosphere of constant change

Physical Demands:

  • Periods of walking, standing, or sitting in an office or field environment for service provision
  • Limited physical effort is required; however, the employee must occasionally lift and/or move up to 15 pounds
  • Ability to operate a motor vehicle, local travel is required
