Change Inc is Hiring an ABI House Manager Near Groton, CT

FLSA Status: This position is considered NON-EXEMPT in relation to the pay provisions of the Fair Labor Standards Act. The work schedule is a ?flex schedule? in that it may require work 1st, 2nd, or 3rd shift based on the needs of the individuals served by ChangeInc. This includes the 2 weekday/rotating weekend coverage policy (to be distributed to you by the ABI Program Coordinator). This position may be assigned to work in various locations/homes. If working hours extend beyond normal business hours, compensation will follow wage and hour guidelines established under the Fair Labor Standards Act as identified in ChangeInc?s Personnel Policies and Procedures.

Reports To: Acquired Brain Injury Program Supervisor

Purpose: The primary purpose of this position is to provide direct care to individuals while supervising service delivery in the house in which you are assigned to. Execution of duties must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern Change Inc and as may be required by the position.

Duties:

  • Provide Independent Living Skills Trainer services to the individual in his/her home and in other community settings.
    • Perform consumer assessments and training as appropriate including:
      • Independent Living Skills assessment
      • Needs assessments.
      • Benefits assessments and any additional assessments as determined necessary.
      • Help and coordinate efforts to modify housing for accessibility and assist in finding funding options.
      • Offers Assistive Technology services including, but not limited to the submission to the state for acquisition, training in usage and assistance in acquiring funding.
    • Train, teach and assist each consumer in learning activities associated with daily living. Implementing treatment plans that focus on individual choice including the following areas:
      • Meal planning, meal preparation and shopping
      • First aid skills, responding to emergencies and symptoms of illness.
      • Money management
      • Self-administration of prescription and nonprescription medication
      • Use of telephone and other public utilities.
      • Development of social, recreational and transportation abilities
      • Specific training plan concerning the development of more appropriate behaviors for consumers displaying inappropriate behaviors.
      • Personal appearance and hygiene
      • Apartment or living environment maintenance.
      • Community resources including but not limited to police, fire department, hospital emergency resources.
      • Right and responsibilities of community living
      • Socialization skills i.e., self-esteem, communication, goal setting, problem solving etc.
      • Educational and employment options
      • Self-advocacy
      • Disability awareness
      • Other identified needs as determined by the consumer and/or interdisciplinary team.
      • Personal attendant Management Training
  • Staff supervision:
    • The ABI House Manager will supervise the daily functions of direct care service delivery in accordance with current rules, Change Inc policies, regulations, and guidelines.
    • See attached document regarding coverage policy, provide coverage as needed when appropriate staffing cannot be arranged.
    • Ensure that all direct care staff assigned to your home comply with the written policies and procedures established by Change Inc. Holding staff accountable for following policies and procedures, disciplinary actions.
    • Meet with your assigned staff on a regular basis, as well as support personnel, in planning the shift's service provision, programs, and activities. Ensure that tasks are completed as directed.
    • Communicate in writing recommendations concerning any changes or concerns in an individual?s condition and other activities as required to the ABI Program Supervisor.
    • Cooperate with other members of the interdisciplinary team to ensure that an individual?s total regimen of care is maintained.
    • Periodically review the individual?s written care plan. Participate in the updating of the consumers written care plan as required. Ensure staff providing care to individuals are providing such care in accordance with the individual?s care plan and wishes.
    • Communicate Change Inc policies and procedures to personnel, individuals, and visitors as appropriate.
    • Upon initial placement and as needed, in service/train house staff on the individual?s specific care needs.
  • Participate in the orientation of new individuals and family members to the agency.
  • Meet and resolve issues with individuals, staff and/or family members. Report significant issues to the ABI Program Supervisor and any other necessary team member.
  • Collect and review required documentation (i.e., Encounter notes, logs, daily progress notes) retrieved from the home along with monthly House Meetings and submit to the ABI Program Supervisor monthly.
  • Maintain individual?s records to include:
    • Daily case/encounter/progress notes
    • Documentation of consumer performance/progress on appropriate internal forms
    • Disposition of incident reports
    • Discharge plan
    • Monthly/quarterly DSS reports
  • Participate in meetings relating to the needs of the consumer to include:
    • Assessment review
    • Annual planning
    • Other interdisciplinary review meetings as determined necessary.
  • Other duties as required or assigned.
