During the fiscal year ending June 30 2016 Chandler Regional provided more than $48 million in community benefit. As a result of Dignity Health’s mission to deliver health care and partner with the community nearly 100000 people in the East Valley were reached through community health services.

Responsibilities

Job Summary :

Assist individuals and communities to adopt healthy behaviors. Conduct outreach for medical personnel or health organizations to implement programs in the community that promote maintain and improve individual and community health.

May provide information on available resources provide social support and informal counseling advocate for individuals and community health needs and provide services such as blood pressure screening.

May collect data to help identify community health needs.

Qualifications

Minimum :

  • Experience working in the community.
  • High School Diploma
  • Ability to work independently set priorities and manage work within timeframes read and analyze technical material communicate effectively verbally and in writing positive mental attitude.

Strong customer service orientation and the ability to handle highly confidential information essential. Must be computer literate.
