We are currently looking for an RN. This is a full to part-time exempt position for someone who has a passion for providing person-centered care at the end-of-life. The ideal candidate is compassionate, independent, energetic, and flexible.

Job Responsibilities:

  • Provides skilled nursing care to patients based on systematic assessment, and as prescribed in the Interdisciplinary Plan of Care, in order to maintain the highest level of comfort and quality of life.
  • Responsible for the timely completion of assessments and the plan of care for all assigned patients.
  • Provides appropriate teaching regarding the care and other related problems of the patient in a language and manner that the patient and family understands.
  • Supervises, assists, and delegates to the Hospice Aide, Medication Aide and/or LPN within their scope of practice.
  • Assumes primary responsibility for case management by authorizing, coordinating and supervising care, as prescribed in the Interdisciplinary Plan of Care, with vendors, outside agencies and volunteers to meet the patient’s needs.
  • Acts as liaison between Hospice Staff, Medical Director, attending physicians, and other health care agencies.
  • Participates in on-call rotation and is accessible to family and patients for 24-hour availability via cell phone during call hours, when assigned to outpatient department.
  • Returns phone calls from patients or families within 30 minutes and, if necessary, makes subsequent home visits within a timely manner when on call.
  • Ensures that patient records are accurate and complete and maintained in accordance with agency policies and appropriate government regulations.
  • Performs patient referrals and assessments in a timely manner, as assigned.
  • Attends deaths and provides post-mortem care in respectful and supportive manner.
  • Attends Interdisciplinary Team meetings, provides appropriate input based upon assessment data, and contributes in developing a holistic Plan of Care to meet the patient and family’s needs.
  • Is knowledgeable in all policies and procedures related to job performance.
  • Other duties as assigned.

Qualifications:

  • Registered Nurse with current Wyoming Nursing license
  • Prefer 1-2 years RN experience

Pay: $35.00 - $45.00 per hour
