CENTRAL FLORIDA HEALTH CARE is Hiring a Behavioral Health Care Coordinator Near Winter, FL

Title: Behavioral Health Care Coordinator
Reports to: Director of Behavioral Health
FLSA Status: Exempt
Personnel Supervised: None
POSITION SUMMARY:
This position serves as the single point of contact for all established CFHC behavioral health patients to coordinate their needs across the various service delivery systems and community supports. The behavioral health care coordinator functions in conjunction with the CFHC’s behavioral health department. The behavioral health care coordinator is essential to the efficiency of utilization of all behavioral health services provided to patients diagnosed with mental health conditions including substance use, co-occurring disorders, social inadequacies, acute and chronic health conditions, and those who are homeless or at risk of homelessness. This position is intended for a highly motivated professional who is passionate about timely coordination of care and access to community resources.
MINIMAL QUALIFICATIONS:
  • Excellent oral and written communication skills
  • Strong interpersonal and organizational skills
  • Positive relationship building skills and ability to engage and motivate health behaviors in diverse populations
  • Ability to quickly identify and prioritize member needs and provide structured and focused support and interventions
  • Two or more years of experience in the administration of community-based services such as behavioral health services, homeless prevention, and coordination, health care services, or public health-related services.
  • High School Diploma/GED required. Bachelor's degree in Public Health, Health Sciences, Public Administration, Psychology, Social Work, Nursing, or related field preferred.
  • Bi-lingual, including English and Spanish preferred.
RESPONSIBILTIES AND PERFORMANCE EXPECTATIONS include, but are not limited to, the following:
  1. Serves as the behavioral health case manager responsible for scheduling, coordinating, and providing internal and outreach efforts for all behavioral health referrals and community assistance programs.
  2. Supports the Director of Behavioral Health (DOBH) in promoting efforts to achieve team, department, and performance goals.
  3. Serves as the liaison for the behavioral health department to improve the utilization of all pharmacies across the CFHC system of care. Behavioral health case manager will meet with all behavioral health patients not enrolled with CFHC’s pharmacy services and provide them with a psychoeducational 15-minute presentation – content at the discretion of to DOBH.
  4. Organize, prioritize, and complete all work assignments by the established deadlines.
  5. Provides consultation to CFHC providers regarding identification of care barriers
  6. Keeps accurate time record of all supportive activities provided via EMR documentation.
  7. Carries out responsibilities in a highly professional manner and in accordance with professional and
    business ethical standards.
  8. Demonstrates highly effective interpersonal communication and conflict management skills.
  9. Ensures that all members of CFHC staff and administration have access to all appropriate information.
  10. Facilitates a positive, open, supportive work environment.
  11. Demonstrates flexibility, creativity, and adaptability in job performance.
  12. Returns patient phone calls and engages in coordination of follow up services.
  13. Maintains clinical records and procedures which are in compliance with State licensing requirements and professional standards.
  14. Is current with and follows Florida Department of Health, standards of conduct, professionalism, and quality of service.
  15. Complies with infection control, occupational and health standards and all rules, regulations and operating plans issued by CFHC, the Occupational and Safety Health Act and JC (Joint Commission).
  16. Understands Continuous Quality Improvement principles and incorporates them into daily work.
  17. Accepts supervision and guidance.
  18. Is flexible when asked to perform other duties.
  19. Maintains strict confidentiality regarding all client and agency information according to policy. A
    professional relationship with all staff, clients and volunteers is maintained at all times.
  20. Will have up-to-date training and certification in Basic Life Support.
  21. Interacts harmoniously and effectively with others, focusing upon the attainment of organizational goals
    and objectives through a commitment to teamwork.
  22. Complies with all company policy and procedures.
  23. Complies with all safety rules and protocols. Reports all accidents, regardless of severity, immediately to supervisor and completes incident/injury report and investigation form, which will be forwarded to Human Resources within 24 hours of incident.
  24. Current on all required training for current year.
  25. Performs all other duties, as assigned by supervisor.
