HOPE for Children and Families, a program within the Central Connection Division of Central Clinic Behavioral Health, coordinates behavioral health services for children from ages 5-adulthood who are involved in at least two of the major service systems within Hamilton County:
Developmental Disabilities
Job and Family Services-Children’s Services Division
Juvenile Court
Mental Health.
Duties:
Evaluate client referrals.
Connect clients to services.
Provide active care management and case coordination.
Discharge transition planning to ensure that clients continue receiving needed services.
Collaborate with all relevant stakeholders to ensure that referral is completed in a timely and seamless manner.
Facilitate team meetings and provide leadership within the team.
Complete referrals to providers.
Monitor client needs and treatment progress.
Continually reassess client to determine appropriate level of care and services.
Minimum Qualifications:
Bachelor’s degree in social work, counseling, or mental health related field
Experience in working with children with behavioral health concerns.
Preferred Qualifications:
Prior experience with child welfare, substance use, juvenile justice, and developmental disabilities.
Licensure in mental health field.
Prior Managed Care experience.
Prior care management/care coordination experience.
