Program Overview
HOPE for Children and Families, a program within the Central Connection Division of Central Clinic Behavioral Health, coordinates behavioral health services for children from ages 5-adulthood who are involved in at least two of the major service systems within Hamilton County:
  • Developmental Disabilities
  • Job and Family Services-Children’s Services Division
  • Juvenile Court
  • Mental Health.
Duties:
  • Evaluate client referrals.
  • Connect clients to services.
  • Provide active care management and case coordination.
  • Discharge transition planning to ensure that clients continue receiving needed services.
  • Collaborate with all relevant stakeholders to ensure that referral is completed in a timely and seamless manner.
  • Facilitate team meetings and provide leadership within the team.
  • Complete referrals to providers.
  • Monitor client needs and treatment progress.
  • Continually reassess client to determine appropriate level of care and services.
Minimum Qualifications:
  • Bachelor’s degree in social work, counseling, or mental health related field
  • Experience in working with children with behavioral health concerns.
Preferred Qualifications:
  • Prior experience with child welfare, substance use, juvenile justice, and developmental disabilities.
  • Licensure in mental health field.
  • Prior Managed Care experience.
  • Prior care management/care coordination experience.
