Job Overview:

A Children’s Case Manager provides flexible, community-based services that are designed to promote the empowerment, recovery, and community integration of children with behavioral health challenges and their families. Employing a person-centered approach, the case manager assists children and their families in navigating various systems, accessing behavioral health services, and coordinating with community and natural supports outlined in their goal plan.

Schedule:

· Full-time, 37.5 hours/week with flexible scheduling to support consumers’ needs including some evening hours.

Responsibilities:

  • Handle a caseload of up to 30 individuals in collaboration with the treatment team.
  • Work with individuals to conduct ongoing strengths assessment and goal planning.
  • Connect children and their families with community resources, assisting them in independently identifying and accessing formal and informal supports and resources.
  • Complete documentation promptly and professionally to meet Central’s standards and state licensing requirements.
  • Attend weekly Supervision and Team meetings.

Qualifications and Skills:

A Children’s Case Manager should meet one of the following criteria:

  • Hold a Bachelor’s degree with Major Coursework in sociology, social work, psychology, criminal justice, or counseling.
  • Possess a high school diploma, completed with 12 semester credit hours in sociology, social welfare, psychology, gerontology, criminal justice, counseling, or other social sciences, along with 2 years of direct contact with mental health consumers.
  • Hold a valid driver’s license with a good driving record

Clearances:

In accordance with state regulations, all prospective new employees/contractors must provide originals for photocopy of their Federal Criminal History Record (DHS FBI Background Check), PA Criminal Background Check, and PA Child Abuse Clearance before commencing the offered position.
