• Serve 12-15 patients per day, affording you the opportunity to focus more time on individualized patient care
• Thrive within a value-based care model, actively improving the quality of life for your patients
• Encounter a diverse caseload, spanning from preventative care to managing chronic conditions
• Dedicate your time to clinical care, including patient interaction and administrative tasks related to patient care

Your Accomplishments are Highly Valued:

• Hold an active, unrestricted NP/PA license in the appropriate state
• 3-5 years of experience as an NP or PA in geriatrics and/or primary care
• Maintain current certification as an NP or PA
• Possess or have the ability to obtain a current DEA license
• Embrace the role's patient-facing nature and participate in Humana/Senior Bridge's Tuberculosis (TB) screening program if selected
• Showcase your passion for contributing to an organization dedicated to enhancing patient experiences

Preferred Qualifications:

• Familiarity with the Value-Based Care model
• Previous experience in outpatient senior care
• Knowledge of HEDIS measures
