We are expanding our foot print and seeking compassionate Physical Therapist to provide home-based physical therapy services. Our Therapist contribute to improving mobility and enhancing the quality of life to our members. Physical Therapist Responsibilities:Travel to and from patients' homes, providing physical therapy as ordered by a physician in accordance with the designated plan of care.Design treatment plans that achieve results through personalized, 1-on-1 individualized care.Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeonsDiagnose movement dysfunction by listening to patients and observing them as they move aboutDevelop quality, personalized, evidence-based care intervention programs that improve outcomesUse hands-on therapy such as massage to ease patients’ pain and provide stimulation to promote healingMaintain patient records by keeping track of goals and progress while adapting treatment plans accordinglyInstructs family members, Home Health Aides, and other caregivers in appropriate physical therapy techniques and what to expect from the recovery processQualifications: Current Physical Therapy Licensure in NY StateGraduate degree or greater in physical therapyProven work experience as a physical therapistIntensive education and clinical expertiseAbility to manage patients with different types of personalitiesCurrent knowledge of treatment practicesExcellent interpersonal communication skills
