Centers Health Care is Hiring a Discharge Planner Near Syracuse, NY

Bishop Rehabilitation & Nursing Center is looking for a Discharge Planner to work in our facility in Syracuse, NY.

DUTIES:

  1. Collaborate with healthcare professionals, including doctors, nurses, social workers, and therapists, to assess patients' medical, emotional, and social needs in order to develop personalized discharge plans.
  2. Educate patients and their families about post-discharge care instructions, medication management, and available community resources.
  3. Advocate for patients' needs and preferences during the discharge planning process, addressing any barriers or concerns that may impact a successful transition from the healthcare facility to the community.
  4. Maintain accurate and up-to-date documentation of discharge plans, patient progress, and communication with healthcare providers and patients' families.

Discharge Planner Required Skills

  1. Excellent communication and interpersonal skills to effectively collaborate and communicate with healthcare professionals, patients, and their families.
  2. Exceptional organizational and time management skills to handle multiple cases and deadlines simultaneously.
  3. Knowledge of healthcare regulations, policies, and resources related to discharge planning.
  4. Proficiency in using electronic medical records systems and other relevant software for documentation and communication purposes.
  5. Ability to work independently and as part of a multidisciplinary team to ensure optimal patient care.

Required Qualifications
