JOB TITLE: Community Pathways Worker, School-Based Early Support Program 

DEPARTMENT:  Family Counseling 

EXEMPTION: Non Exempt 

REPORTS TO: School Outreach Liaison 

SUPERVISES: N/A 

PURPOSE OF THE POSITION: This is an excellent position for someone who wants to develop their career in social work or counseling while working with families and schools. This is a new position in the School-Based Early Support Program, a new preventive services model that enhances school-based partnerships. The Community Pathways Worker provides services to 8 families at a time across four different partner schools in Brooklyn School District (15 or 20) focused on case management and referrals to needed resources. The Community Pathways worker also will work with the School Outreach Liaison to implement the Families and Schools Together (FAST) program. 

I. SPECIFIC RESPONSIBILITIES include but are not limited to 

  • Support the School Outreach Liaison with school partnership efforts, developing crucial relationships with partner schools and families 
  • Responsible for providing case management to 8 families across four schools in Brooklyn School District 15 or 20 
  • Participate in the implementation of the Families and Schools Together (FAST) program including co-facilitation of multi-family groups 
  • Assist to codesign and coordinate school-based events 
  • Collaborate with CFL program staff, school staff, other community providers, and multiple systems (housing, public assistance.) to meet the needs of families 
  • Provide direct advocacy and support to families by accompanying them to access services in the community and connecting them with needed resources 
  • Conduct home visits as needed 
  • Complete required documentation and data entry 
  • Attend staff meetings, supervision, trainings, and meetings within partner schools 
  • Build relationships with community-based service providers and other key community stakeholders 
  • Collaborate with all CFL programs to ensure an integrated approach to serving families 

 II. QUALIFICATIONS 

  • High School diploma or equivalent with extensive experience working with families and groups; OR
  • BA/BS/BSW with relevant experience 
  • Bilingual in Spanish Mandarin, or another language spoken in school community, required 
  • Parent/Caregivers with lived experience navigating resources for their families or special needs children, and/or volunteering in schools encouraged to apply 
  • Experience working in, or in partnership with, schools preferred 
  • Groupwork skills and experience preferred 

 III. RELATIONSHIP WITH OTHERS 

  • Build and maintain close, daily interactions with co-workers, supervisors, school staff and families 
  • Effectively communicate with agency staff, school staff, key stakeholders and families 
  • Excellent engagement skills, meeting families where they are at and forging mutual respect 
  • Experience or interest in social group work 
  • Have a strong sensitivity to cultural differences present among staff and clients within our organization 
  • Possess a strong belief in people’s ability to grow and change 

IV. WORKING CONDITIONS 

  • Work is primarily in a school setting with some office work 
  • Some time is spent at a desk using a computer 
  • Attendance at various meetings at schools, the agency, and in the community
  • Regular travel among four schools located in the school district
  • Travel in Brooklyn, New York to meet with families, including home visits 
  • Some evening and weekend work is required 
  • Fast-paced, high-energy, and highly collaborative environment 
  • This is a full-time, in-person position 

V. SCOPE OF RESPONSIBILITY

The Community Pathways Worker provides case management and group facilitation in the context of the school community. They are essential to developing effective school partnerships through intentional engagement strategies and supporting events and activities within the schools. They are an ambassador for CFL, interacting at all times with respect and kindness, generosity, and solution-focused problem-solving.

This is a Full-time non-exempt position at 35 hours per week.
