Center for Elders' Independence is Hiring a Home Care Scheduler Near Oakland, CA

The Position:Under the direct supervision of the Home Care Supervisor, the Home Care Scheduler supports the Home Care team by maintaining an effective, flexible scheduling program; providing training and support for the Personal Care Attendants (PCAs) including assistance with orientation; consulting on complex cases; and developing and maintaining data to track ongoing quality improvement efforts in the Home Care department.
The rate range for the Home Care Scheduler at Center For Elders Independence is $23.13 - $ 34.70/hr. Wages/salary are based on the market for the (title) position, as well as experience, skills, abilities and work history
Duties and Responsibilities:
  • Prepare and maintain the schedule according to participant needs, Home Care Coordinator communications and ensures services provided/scheduled are in adherence to service plan received by the Home Care Coorindator or relevant IDT member. Determine best schedule for participant based on location, language and PCA availability. Communicate changes and scheduling issues with the Transportation department, Home Care Coordinators, participants, families and other staff.
  • Report pertinent participant events, missed visits, or “No Contacts” to assigned Home Care Coordinator and direct supervisor.
  • Cancel Home Care scheduled for participant who is going into a SNF, RCFE, or who is admitted to a hospital in a timely manner. Ensure Home Care will be ready with staff in place for care when participant returns home.
  • Assess home care staffing needs and report to your supervisor and Director of In Home Services.
  • Ensure electronic service plans are accurate, current and available in, and match between home care department and CEI’s Electronic Medical Record (EMR) systems.
  • Communicate personal care and chore service schedules to home care coordinators and other staff, participants and families as appropriate including changes to the schedule.
  • Provide non-clinical orientation and support training of new hires and Home Care staff as assigned. Train staff in office procedures and related software used by the home care department.
  • Follows all CEI Policies and Procedures and OSHA safety guidelines.
  • Identify learning gaps and education needs of the home care team and make recommendations to address them at departmental meetings.
  • Provide phone support to staff in the field; including trouble-shooting on assigned weekends.
  • Run daily and monthly reports as required. (such as Home Care visits, “No Contacts”, cancellations, hours worked, etc.). Present results and relevant data to supervisor and / or Home Care Administration team.
  • Calculate Home Care hours required for new and current participants.
  • Participates in the agencies quality assurance performance improvement program (QAPI) and any audit or compliance related requirements as assigned.
  • Complies with all agency training requirements.
  • Works in collaboration with other Home Care Supervisors on projects and assignments as required.
  • Maintains a courteous, helpful and professional attitude on the job. Displays a willingness and ability to be responsive in a warm and caring manner to all customer groups.
  • Performs other duties as assigned in a positive and helpful manner.
Qualifications:
  • Must be available weekends and evenings to work on-call.
  • High school diploma or GED required.
  • Certified Nursing Assistant certification preferred.
  • Current Basic Life Support CPR card is required
  • Strong computer software skills, including proficiency with Microsoft Office products (Word, Excel, etc.). Ability to master new software programs and databases.
  • Two years previous experience in a scheduling or administrative capacity, preferably in health care environment preferred.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers and all customers. Possesses good telephone/customer relations skills.
  • Strong verbal and written communication skills.
  • Exercises good judgment, discretion, initiative and flexibility.
  • Able to plan, organize and meet deadlines, taking into account changing priorities.
  • Able to work in multi-cultural and multi-disciplinary setting.
  • Demonstrated ability to work independently with minimum supervision.
  • Minimum 1 (one) year of experience working with frail, low income elderly is preferred.
  • Ability to work independently with minimal supervision and as part of a multidisciplinary team is required.
  • Bilingual is preferred but not required. Proficient with oral and written English is required.
