Under the direction and supervision of the Day Center Supervisor, the Geriatric Aide II provides direct services to CEI participants at the Day Center including physical assistance, attendant care and psychosocial support to CEI participants and immediately reports any concerns to ensure that participants' needs are addressed. The Geriatric Aide II demonstrates knowledge and skills necessary to participate in providing care to frail elders according to assigned responsibilities and PACE Program standards, while ensuring participant satisfaction and retention. The Geriatric Aide II Assists the Day Center Supervisor in the monitoring of Geriatric Aide I daily assignments, orientation and training of new day center Geriatric Aides The Geriatric Aide II is responsible for maintaining proper level of supplies in the center for participants take-home supplies.
Compensation range for the Day Center Geriatric Aide II role at CEI is $20.50-29.97. Offers based on level of experience/education as it applies to the requirements for the position.

Duties and Responsibilities:
  • Assists in the scheduling and monitoring of the GA daily assignments for completion in conjunction with the Day Center Supervisor. Assigns extra duties as needed.
  • Oversees functioning of GA's under the supervision of the Day Center Supervisor and/or in the absence of the Day Center Supervisor.
  • Assists in orientation and training of new Geriatric Aides.
  • Monitors that participants are ready to be on time for appointments at the center and for outside appointments leaving the center.
  • In conjunction with the Day Center Supervisor, trouble shoots any assignment gaps and makes adjustments in the assignments as necessary. Actively participates with recommendations for improving participants care and reports relevant information on a daily basis to day center team. Communicates changes in participants care plans to GA's and monitors appropriate implementation.
  • Promptly reports changes in participants' condition needs to the Day Center Supervisor, Center Director or Clinic RN or designee.
  • Attends all scheduled in services as assigned. Must attend a minimum of 6 CEI in services per year.
  • Observes, records and reports to the Day Center Supervisor the satisfaction of the participant to the activity program, and the nutrition program for program improvements.
  • Provides toileting assistance to dependent participants including prompting and escorting to bathroom, proper use of lifting devices or transfer techniques, assisting with hygiene, and changing soiled clothing or incontinence supplies.
  • Provides showers in the center to participants according to their care plan.
  • Washes participants laundry in the center according to their care plan.
  • Records services rendered in the electronic health record or other designed forms as needed.
  • Assists with all general activities at the center such as greeting participants and escorting ambulatory and mobility impaired participants to and from the van. Observes and reports promptly any changes in ambulation and/or transferring.
  • Ensures visitors' needs are met and are directed to the appropriate area for the purpose of their visit.
  • Accompanies participant(s) at medical appointments, providing assistance and monitoring, including toileting and cognitive redirecting throughout appointments to ensure the safety of the participant at these appointments. Responsibility could include driving personal vehicle to medical appointments as directed to meet the participant at the appointment.
  • Assists with the individual and group activities in art, music, crafts, discussions, physical, fitness, psychosocial function and other activities as necessary.
  • Assists with nutrition service program by setting up for, and serving snacks and meals. Provides feeding support to rehabilitation staff for participants needing assistance.
  • Responsible in keeping center in a clean and orderly condition, including showers, bathrooms, kitchen, and activities areas.
  • Responsible to ensure that all participants on "Wander Guard" list have device on. Checks and maintains door alarm batteries and settings routinely.
  • Responsible for maintaining proper level of supplies including any required ordering and submitting to Day Center Supervisor or designee for approval.
General Duties and Responsibilities:
  • Acts as a client advocate by upholding, and reporting any abuses of the Participant Bill of Rights & Responsibilities.
  • Responds to participant complaints in a timely manner and understand the basic procedures for receiving and documenting grievances in order to initiate the appropriate process for participant concerns.
  • Completes required paperwork accurately and on time.
  • Demonstrates dependability through consistent and punctual attendance to scheduled work hours and submits timely and accurate timesheets.
  • Maintains a courteous, helpful and professional attitude on the job.
  • Displays a willingness and ability to be responsive to all customer groups, including staff, participants and their families.
  • Complies with all agency training requirements.
  • Maintains professional affiliations and any required certifications.
  • Provides a safe environment for participants and self by displaying awareness of safety hazards and reports and/or reverses hazards.
  • Participates in Quality Improvement program as required.
  • Follows all OSHA safety guidelines and CEI Policies and Procedures.
  • Maintains the confidentiality of all participant information according to HIPAA requirements.
  • Performs other duties as required or requested in a positive and helpful manner.
QUALIFICATIONS:
  1. High School Diploma or equivalent preferred.
  2. Certified as a Certified Nursing Assistant in the State of California required.
  3. Minimum one year working as a C.N.A. in a geriatric setting.
  4. Knowledge of the physical, mental and social needs of the elderly.
  5. Good written and verbal communication skills, with ability to maintain accurate records.
  6. Excellent customer service skills.
  7. Ability to work with multidisciplinary team.
  8. Good organization and scheduling skills
  9. Supervisory ability and skilled in establishing and maintaining effective working relationships with patients, co-workers, medical staff, and the public.
  10. Skilled in identifying and recommending problem resolution.
  11. Ability to work independently with minimum supervision
  12. Ability to communicate clearly and effectively
  13. Ability to work in a multicultural and multidisciplinary setting.
  14. Ability to work with different disciplines.
  15. Current CPR certification and First Aid training.
  16. . A valid California driver's license, current vehicle insurance, and access to a reliable vehicle preferred.
