With programs and services for the entire family, our team provides individual and family therapy, play and group therapy, mental health evaluations, psychological testing, and more.

POSITION DESCRIPTION

The Care Coordinator is responsible for connecting clients to appropriate community resources and monitoring/evaluating outcomes. This includes meeting with a case load of up to 20 clients each month, to evaluate concerns, needs and psychiatric emergencies as needed. This is a full-time, exempt position, reporting to the Coordinator of Community Support Services or their designee.

  • Schedule – day shift and two evenings/wk
  • Pay - $40,000 - $42,000/yr, based on experience

EDUCATION AND WORK EXPERIENCE REQUIREMENTS

  • BA/BS degree in mental health or human services fields (i.e. human services, psychology, counseling, social work, sociology, etc.).
  • Ability to handle stressful situations.
  • Understanding of cultural diversity.
  • Ability to observe and accurately report the functioning of children and families.
  • Valid driver’s license and vehicle insurance
  • Successfully complete State and Federal background investigation and drug test

KEY JOB RESPONSIBILITIES

  • Maintain an active case load of a maximum of 20 clients per month.
  • Conduct a minimum of two face to face meetings per month with each client.
  • Complete documentation and maintain files per COMAR regulations within allotted time frames.
  • Conduct intakes, comprehensive assessments, and re-assessments
  • Develop Plan of Care (POC) that identifies goals and interventions.
  • Provide supportive counseling and advocacy for clients; engage clients in the problem-solving process.
  • Advocate on the behalf of the client with external agencies, request supporting documentation from therapists, schools, doctors, or other agencies
  • Represent the program to outside agencies, including the Local Behavioral Health Authority.
  • Complete a minimum of 40 hours of training in the area of case management or topics related to client care/self-care.

Pay: $40,000.00 - $42,000.00 per year
