Duties and Responsibilities

• Coordinate appropriate member assistance relating to transportation, scheduling appointments for mental health services or behavioral health treatment services, assistance with benefit questions, member coaching relating to no-shows, disruptive behavior, and other tasks as requested by Behavioral Health Department staff.
• Document coordination efforts in appropriate systems and communicate outcomes of interventions with referring providers and other departments such as Case Management within expected time frames.
• Make referrals to other CenCal departments for necessary involvement i.e. Pharmacy, Care Coordination, Disease Management, Health Education, Population Health and Member Services, as appropriate or as requested by Behavioral Health Services Managers or Director.
• Active participation and support to department’s compliance and quality activities
• Adhere to all CenCal Health Policies and Procedures and meet required performance standards
• Other duties as assigned by the Director of Behavioral Health or the Behavioral Health Care Coordination Manager.
