General Duties and Responsibilities of the BH Care Manager :

  • Maintain a high level of ethical conduct while complying with HIPAA, Privacy, and Confidentiality laws and regulations
  • Adhere to Health Plan, Behavioral Health and Health Services policies and procedures
  • Be abreast on clinical knowledge related to mental health or developmental disabilities
  • Effectively communicate, verbally and in writing, with providers, members, vendors, and other health care providers and in a timely, respectful and professional manner
  • Function as a collaborative member of Health Services’ multi-disciplinary team
  • Identify and report quality of care concerns to management and as directed, to appropriate CenCal Health department for follow up
  • As required, actively participate in the implementation, assessment, and evaluation of quality improvement activities as it relates to job duties
  • Adhere to mandated reporting requirements appropriate to professional licensing requirements
  • Comply with regulatory standards of governing agency
  • Be positive, flexible, and open toward operational changes
  • Attend and actively participate in department meetings and external community meetings.
  • Actively participate in the development, implementation and the evaluation of department initiatives with the intent to assess any measurable improvements to member’s quality of care 
  • Embrace innovative care strategies that are build value based programs

Behavioral Care Manager (Mental Health) Responsibilities:

When assigned to the Mental Health Team, in addition to the General Duties and Responsibilities noted above, Behavioral Health Care Management responsibilities included but are not limited to:

  • Facilitate and assist members with accessing mental health care through care coordination and education.
  • Participate in interdisciplinary care team meetings to share information and update and inform care plans.
  • Accurate and timely determination of member level of care based on assessment or survey results.
  • Effectively communicate and educate members about the health care delivery system and health plan benefits and carve out benefits related to behavioral health services and substance use services
  • Assist members with navigating through CenCal Health healthcare delivery system to meet identified or possible needs.
  • Empower members by providing community resources, educational materials, and self-managing tools
  • Ensure documentation is accurate and in compliance with regulatory requirements and accreditation standards.
  • Document accordingly in Case Management treatment plans updates on Mental Health Treatment goals and other applications as designated.
  • Collaborate with County Mental Health, contracted agencies and community based organizations to provide supportive services when needed and act as the CenCal Health liaison to both Counties.
  • Coordinate timely care transitions from SMHS to/from CenCal Health Behavioral Health network.
  • Facilitate and/or attend assigned community meetings that may include County Department of Behavioral Health, FQHC’s, Community Health Clinic, and Regional Center to support member access to care and evaluate the current delivery system.
  • Manage cooperatively the “By Name” list of Members transitioning to/from SMHS (including SUD).
  • Effectively communicate verbally and in writing with primary care providers and other health care providers involved in the care of the member
  • Utilize Solution Oriented approaches in communication with providers, stakeholders and members.
  • Refer members to case management as appropriate.
  • Provide timely and detailed reports/briefings to supervisor as assigned.
