The Director of Social Work in a long-term care setting plays a crucial role in ensuring the well-being and quality of life for residents. This role entails managing the admissions process, discharge process, ensuring compliance with regulations, and fostering positive relationships with residents and their families.
- Collaborate with other department heads to ensure the integration of social services into the overall care plan for residents.
- Conduct comprehensive assessments of residents' social, emotional, and psychological needs.
- Develop individualized care plans in collaboration with residents, their families, and the interdisciplinary care team.
- Regularly review and update care plans to ensure they reflect residents' changing needs.
- Advocate for residents' rights and ensure their voices are heard in the care planning process.
- Maintain accurate and up-to-date records of social work assessments, interventions, and care plans.
- Ensure compliance with relevant state and federal regulations, as well as industry standards.
- Work closely with healthcare professionals, including physicians, nurses, and therapists, to ensure holistic care for residents.
- Collaborate with external agencies, government entities, and advocacy groups to enhance the support network available to residents.
- Coordinate and manage the entire admissions process, from initial inquiry to resident move-in.
- Serve as the primary point of contact for prospective residents and their families, providing information about the facility, services, and admission criteria.
Qualifications:
- Bachelor's degree in Soical Work or Related field.
- Previous experience in admissions, marketing, or a similar role in a long-term care setting.
- Knowledge of state and federal regulations governing long-term care admissions.
The Director of Social Work in long-term care must possess strong leadership, customer service, and interpersonal skills. Additionally, a thorough understanding of social work principles, ethics, and relevant regulations is essential for success in this role.
