Cayuga Centers is hiring immediately for Youth and Family Skills Coach

Location: Utica, NY

Earn from $20.00 to $22.00 per hour depending on education

About the Program and Position:

The Cayuga Centers’ Upstate New York Treatment Family Foster Care (TFFC) Team is looking for a clinician/therapist who is passionate about supporting hard-to-place youth in the foster care system. We wrap services around the youth, their family/kin/caregivers, and the foster parents to address trauma related symptoms and reunify youth with their family at home or discharge to a permanent home as soon as possible. As a Youth and Family Skills Coach, you will work on a dynamic multidisciplinary treatment team.

How you get to use your skills…

  • Conduct life skill development assessments, using approved assessment methods
  • Following assessment and supporting Treatment Plan goals, create a Life Skills Development Plan and provide psychosocial rehabilitation activities to meet the youth’s needs, as specified by the assessment and Treatment plan, and to support developmentally appropriate functioning of the youth.
  • Complete and document required credit checks and reviews with youth
  • Complete OCFS transition plans with youth
  • Provide life skills coaching to youth at least 1 hour per week, one or more times per week, as determined by the team and an assessment of youth need
  • Connect the youth with appropriate community activities and resources to teach and practice learned life skills
  • Immediately report and respond to emergencies, such as runaways, accidents, or medical problems
  • Provide home, community, and school based behavioral stabilization/coaching/support as necessary and outlined in Behavior Modification Plans
  • Provide hourly respite as designated by the Treatment Team
  • Consult with the youth’s Treatment Team, participate in supervision and act as a part of the youth’s Treatment Team, whose ultimate goal is to ensure that every one of the placed youth is living in a nurturing, safe, and permanent family as quickly as possible. This individual will also assist with family time/visitation.
  • Develop Family Time (Visitation) Plans as specified by the Treatment Plan
  • Plan, supervise, and document family time that takes place between youth and their families/relatives.
  • Utilize the Coaching Family Visits approach to supervising, preparing for, and de-briefing family time
  • Provide transportation for youth as needed
  • Communicate respectfully with youth, foster parents, biological parents & colleagues
  • Document all contact with youth and families on Connections and Netsmart within the 72 hour requirement
  • Provide prevocational services to youth, as needed
  • Provide habilitation services to youth, as needed
  • Attend treatment related meetings such as school meetings, Service Plan Reviews, Clinical Team meetings, etc
  • Attend required training to enhance skills and knowledge as it pertains to their role as a Youth and Family Skills Coach

Schedule: Non-Exempt

  • 40 hours per week
  • Schedule will be provided based on program needs, evenings and weekends required
