Job Description:

Earn $48,500/annually, plus 3 weeks vacation, 10 calendar holidays (Includes 1 floating holiday), 24 hours personal time, and up to 80 sick days.

The successful candidate will provide Preventive Case Planning Services to children, youth, and families in this new contract program. Join us as we forge a new partnership with the county and provide prevention services!

The Preventive Case Planner will work closely with the county, the Preventive Case Planning Services Team, and other service providers to ensure children, youth, and families’ needs are met safely! Bring your experience, compassion, and energy to the team and make a difference in the lives of children in our community!

What you will do….

• Engage children, youth, and families in case planning activities to prevent out of home placement/support youth returning home from placement

• Conduct assessments of strengths and needs

• Develop and carry out service plans to support youth and families in goal achievement

• Assist children, youth, and families in achieving timely and lasting permanency
