Catholic Charities West Michigan is Hiring a Case Manager, Post Release Services Near Grand Rapids, MI

Position Summary:
As an Unaccompanied Children (UC) Post Release Services (PRS) Case Manager, is primarily responsible for providing casework and supportive services to children released from Office of Refugee Resettlement (ORR) care and are residing with sponsor families in the community.
This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements. Bilingual (Spanish and English) ability required.

Annual Salary: $45,000.00

Essentials Duties and Responsibilities:
  • Conducts in-person home visits and phone contacts with the dual purpose of assessing safety and appropriateness of child’s placement with their caregiver (sponsor) and connecting child to services in the community to address risk factors.
  • Ongoing case management with a focus on screening and making appropriate referrals to address each child’s needs in the following domains: placement issues, safety issues, education, physical and mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, child enrichment activities.
  • Provides crisis intervention services directly, as needed, or connect family to services to ensure minor’s safety and well-being in a crisis
  • Submits detailed and thorough documentation in compliance with United States Conference of Catholic Bishops (USCCB) and Office of Refugee Resettlement (ORR)
  • Maintains case file documentation in accordance with requirements of USCCB and ORR
  • Adheres to confidentiality standards set forth, when gathering documenting, consulting, or consolidating information necessary for the case management assessment and the plan of care
  • Ensures regular contact with each child’s family when possible, and established service providers to communicate case planning and progress. Involves going into clients’ homes.
  • Completes required reports according to outlined timeframes
  • Attends regularly scheduled supervision with direct supervisor to consult on cases, to review case plan and direction, and to evaluate client participation and progress.
  • Maintains effective communication with community agencies or individuals involved in service provision.
  • Maintains regular and timely communication with supervisor around any issues concerning the child’s safety, stability, or well-being in the placement
  • Attends internal and external training and apply newly gained knowledge in providing services.
  • Drives for Agency business.
  • Performs other special assignments at supervisor’s request.

  • Ability to work effectively on a multi-disciplinary treatment team.
  • Ability to build rapport with clients to help them adjust to new life circumstances.
  • Ability to provide excellent and compassionate customer service.
  • Ability to work independently and exercise good judgement, and a high level of confidentiality.
  • Excellent verbal and written communication skills.
  • Flexible scheduling
  • Mature and stable judgment as well as sensitivity to various cultures and the unique history of refugees.
  • Knowledge of the National Association of Social Work Code of Ethics and the NASW Standards for the practice of direct social work.
  • Computer and typing skills sufficient to perform essential job functions.
  • Ability to relate to diverse populations and cultures while communicating with clients and the public in a courteous and effective manner.
Recommended Employment Qualifications:
Education:
A bachelor's degree in social work or a related field in human services is required.
Experience:
Bilingual Spanish/English fluency is required.
A minimum of one year of experience in child and family services is preferred.
A minimum of one year's experience working with children and families from diverse cultures with sensitivity to cultural difference and norms is strongly preferred.
