Position :Refugee Health Case Manager (Burmese Refugee Health Collaborative) Reports to: Refugee Services Supervisor Supervises :None Department: Refugee Services FSLA Status :Non-exempt, Part-time as needed Position Summary :Provide a wide range of professional services to meet the needs of Burmese nationals resettling in the Fort Wayne area within 1 year of their arrival date in the U.S.. Provide interpretation, transportation, and other services as needed. Works in collaboration with IU Health Foundation Funded Burmese Refugee Health Collaborative. Essential Duties/Responsibilities :
Along with the refugee resettlement services, the Refugee Health Case Manager will assess and work with refugees in a manner that is respectful to the refugees’ priorities, allowing for the most successful resettlement possible.
Coordinate appropriate follow-up care as clients receive referrals.
Ensure the coordination of available preventive health services and the availability of ancillary services for Burmese nationals.
Assist clients in addressing barriers to access of health services, including but not limited to interpretation, transportation, health insurance and benefits, making appointments, understanding provider instructions, medications, and responding to medical-related mail.
Assist clients in accessing services related to social determinants of health such as healthy food and benefits such as SNAP and WIC, health insurance enrollment (Medicaid, HIP, marketplace, etc.).
Coordinate physicians’ appointments and other follow-up services as necessary.
Provide culturally sensitive and appropriate interpretation and translation for non-English speaking clients.
Comply with grant, funding, and audit requirements for the program.
Participate in educational and training activities to improve professional and job-related skills.
Assume additional appropriate duties and responsibilities as required.
Qualifications:
High School diploma, associate or bachelor’s degree preferred.
Bilingual in English and Burmese
Some knowledge of public and private social service providers in the Ft. Wayne community.
The ability to represent the agency in different settings, and to work with people from a wide variety of cultures.
Good communication skills, English verbal and writing skills.
Ability to work as a translator with knowledge of one or more languages.
Conditions Of Employment:
Have a valid driver’s license with a clean driving record.
If your position requires you to transport clients, staff members, or volunteers with your personal vehicle as needed, it is required that you have a reliable vehicle with personal vehicle liability insurance coverage per person and per occurrence.
If your position does not require you to transport clients, staff members, or volunteers with your personal vehicle, or such transportation is restricted to use of agency vehicles, your personal vehicle liability insurance coverage must be per person and per occurrence.
Successful completion of federal and state background checks
Successful completion of child welfare screening
Successful completion of drug and Mantoux (TB) screens
