Supervision

• Oversees and manages the care, safety, and well-being of each
• Maintains “line of sight and sound” supervision at all times and when applicable, provides one on one
• Ensures that ratio is maintained at all
• Log headcounts every 15 minutes and document changes to
• Physically count minors when transitioning throughout the
• Ensures all minors are accounted for at all times while off (i.e. field trips, appointments, etc.)

Travel Local Travel

• Transport minors safely to appointments including but not limited to; medical, dental, religious services, field trips, court hearings, and attorney’s offices utilizing agency vehicles.
• Upon return, provide staff with necessary documentation Out of Town Travel
• Overnight and out-of-town travel with minors is required for reunification or
• If traveling by air, at the departure airport, check in the minor at the ticket counter with a copy of proper
• Verify, confirm, and release the minor to the sponsor at the identified
• Follow up with Program Director or Designee if the minor is unable to be

Intake and Discharge

• Assist with the intake and discharge

Service Delivery

• Weekly facilitates two telephone calls (10 minutes each) to safe family members and/or sponsors in a private
• Facilitates regular contact with safe family members or other support systems through letters and program
• Participates in a family-style meal with the minors, except when age or special dietary restrictions dictates
• Report unusual behavior in required
• Complete Shift Summary prior to the end of the
• Daily conduct random room checks using the required form and report findings to
• Daily document recreational activities and religious services for each
• Actively participate in interdisciplinary team

Risk Management and Quality Assurance:

• Performs PQI functions as assigned by
• Participates with ongoing safety inspections
• Participates monthly in the fire drill
• Timely and accurately submit Unusual Incident Reports and include all relevant documentation required by regulatory bodies.
• Must be available to work beyond your regular schedule in crisis situations or if there is a declared agency

Additional Duties:

• Participates in all related trainings, in-services, and staff meetings as required by COA and funding /licensing
• Incumbent will comply with all policies, procedures and requirements necessary to perform the functions of this
• Maintains a valid driver’s license and provides transportation utilizing agency vans and supervision when

Physical Demands:

• Ability to lift and carry up to 25
• Ability to walk, bend, stoop, stretch, squat, kneel, push, pull, and sit
• Education :
• Be at least 25 years of age
• High School Diploma or GED
• 1-year minimum employment experience in the child welfare field working with children and or adolescents
• Must have a valid Florida driver’s license and 3 years of clean driving record history or less than 6 points in their license within the last 3 year
• Bilingual – English/Spanish required
