Catholic Charities of San Francisco is Hiring a Case Manager Near San Rafael, CA

ORGANIZATION SUMMARY:
Catholic Charities is one of the largest, oldest, and most comprehensive nonprofit human services organizations in Northern California, annually reaching more than 60,000 vulnerable individuals of all faiths in San Francisco, San Mateo, and Marin Counties. With a staff of approximately 450, reaching peaks of nearly 600 with seasonal hires, a budget of $54.3 million, and a current governing board of 27, this venerable, fully-accredited, independent 501(c)(3) nonprofit addresses some of the most pressing community needs – homelessness, generational poverty, immigration, inequality, hunger, aging in isolation, and communicable disease - to name a few – with compassion and services of highest quality.
PROGRAM & POSITION SUMMARY:
Catholic Charities St. Vincent’s ORR provides residential care and placement support to male unaccompanied minors ages 6 -17 in San Rafael, CA following the guidelines set forth by the Office of Refugee Resettlement. This program provides a safe, nurturing, and supportive environment that serves the whole child through comprehensive medical and mental health care, nutrition, educational programming, counseling, legal assistance, case management, religious access, and age-appropriate acculturation and recreation.
With an emphasis on strengths- and evidence-based, client-centered, and trauma-informed care, delivered by an experienced bilingual staff, we will provide a secure and enriching environment for all youth. An individualized service plan will move each child toward a successful safe and timely release and permanent home placement while building a robust support framework that will follow the child upon entering their new community setting.
The Case Manager is responsible for coordinating case management services. The Case Manager’s tasks shall include development of in take assessment, individual service plan, family reunification and discharge. The Case Manager shall also ensure that all services provided to each child are properly documented in each child’s case file.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Demonstrate Leadership:
  • Promote professional image of the agency through interpersonal interactions and appearance.
  • Develop and maintain professional and supportive relationship with all agency staff, volunteers, and community.
  • Make decisions, taking into considerations, the needs of individuals as well as the organization.
  • Demonstrate a team approach with peers and colleagues.
  • Discuss a difference of opinion without becoming upset, abusive, or otherwise defensive.
  • Acknowledge limitations, uncertainties, or mistakes.

  • Complete assigned tasks within the time frame allotted.
  • Provide feedback when difficulties arise in completing tasks.
Complete Required Documentation:
  • Participate in weekly case management team meetings to prepare for weekly case review meetings by discussing updates on all cases with case management team.
  • Participate in weekly case review meeting to discuss cases with case coordinator.
  • Complete an Initial Intake Assessment within 24 hours of placement.
  • Maintain Case Management Update notes, Case Manager Checklist, and Running Log. Submit Case Management Update notes and Case Manager Check list to Program Manager weekly.
  • Complete Case Review on portal and Monthly Progress Reports every 30 days(or 15 days Case Review, when applicable) on all assigned clients.
  • Complete any necessary referral forms and all other forms as necessary and requested.
  • Ensure that all required documentation is submitted to Program Manager, or filed in master file, in a timely fashion.
Family Reunification:
  • Identify and contact potential sponsors within 24 hours of intake.
  • Send Family Reunification Packet to identified sponsor as soon as possible, and encourage sponsor to complete FRP and return it along with requested documentation within 7 days of receipt.
  • Conduct phone interviews with sponsor at least 4 times a week to maintain communication. During phone calls assess strengths/needs of sponsor and ascertain information necessary for a safe reunification.
  • Review information regarding sponsor with case coordinator during weekly meetings to work in collaboration with case coordinator to determine recommendations about reunification.
  • Review all documentation on portal before requesting release.
  • Coordinate the development of the Individual Service Plan (ISP)
Collaboration with Sponsor:
  • Demonstrate dignity and respect for sponsor.
  • If approved, schedule and coordinate sponsor visits to campus in collaboration with team.
  • Maintain communication regarding sponsor visits, schedule of telephone calls, etc.
  • Assist sponsor with identifying natural supports.
  • Assist in creating weekly approved call logs (at least 2 calls per week per ORR regulations). Review call summary logs weekly.
  • Coordination of Services:
  • Facilitate referrals to the appropriate service as needed.
  • Maintain ongoing communication with personnel from those services involved in the child’s care.
• Provide support and assistance for the child’s positive participation in group living and community activities. • Become familiar with community resources and natural supports.
  • Maintain direct contact, having at least one scheduled contact per week to discuss reunification, per ORR regulations.
Assist in Discharge Planning:
  • Assist in developing a discharge plan (ORR safety plan) which identifies appropriate supportive services and natural supports, when needed.
  • Schedule discharge exam with nurse to take place no sooner than 24 hours prior to discharge.
  • Make appropriate referrals to the identified post release service agencies as necessary. Make referrals to home study providers when indicated.
  • Ensure that post release service provider has confirmed acceptance of case before UC is discharged.
  • Inform ICE and VERA about the change of address of UC (at least 24 hours before discharge).
  • Confirm the verification of release and other documents are signed by sponsor and maintained in master file.
  • Prepare other necessary documents for sponsor.
  • Collaborate with clinician to complete DPW Termination Review and Discharge Summary with in 3 days of discharge.
Utilize a team approach to the care of the children, support and collaborate with program staff:
  • Respect youth care workers’ role.
  • Collaborate with youth care workers, medical department and other departments in meeting the child’s medical, emotional, social, physical and spiritual needs.
  • Meet with the children’s youth care workers at least twice per month to review progress and areas of concern during cottage meetings.
  • (Attend cottage team meetings at least two times per month.)
  • Review cottage communication logs daily.
Professional Development:
  • Participate in team meetings, supervisory meetings, and other program meetings.
  • Attend mandatory trainings and other trainings to enhance professional development, participating in a minimum of 40 hours of required refresher trainings per calendar year—and assure that CPR and CPI training are completed annually within one year of the last date in which the employee last took them.
  • Promptly report; 1. any civil or criminal convictions, charges, arrests, investigations or adjudications— that may arise since staff member’s last background investigations, including but not limited to, 2. having engaged in or attempted to engage in sexual abuse, sexual harassment or inappropriate sexual behavior; having been civilly or administratively adjudicated to have engaged in or attempted to engage in 1. or 2. above.

QUALIFICATIONS:

Education & Experience:
  • Master’s degree is preferred or a Bachelor’s Degree in the behavioral sciences.
  • Behavioral sciences, human services, or social service fields and 3-5 years of progressive employment experience in the human service fields.
  • Child welfare and/or case management experience is strongly encouraged.
  • Must be bilingual
