Cassia, a nonprofit, faith-based affiliation between Augustana Care and Elim Care, is hiring a RN, Assessment Nurse to join our team. In this role, you will support the transition of potential residents wanting to move into a Cassia facility. This person will provide clinical expertise and collaborate with the onsite team, marketing, operations, and home office clinical team to help drive the success of occupancy. The assessment nurse will complete pre-admission assessments within the community (TCUs, hospitals, homes, etc).
The assessment nurse must be able to travel throughout the day to multiple locations as needed. We are looking for someone who is self-motivated and has excellent communication and customer service skills.This is a Full-Time, benefit eligible position.Responsibilities:

• Develop and maintain collaborative relationships across Cassia.
• Effectively use program tools to complete assessments, notes, and documentation of process.
• Complete and analyze reports for completion of tasks and projection of occupancy.
• Display a courteous attitude and respect for all residents, families, and staff.
• May work at sites as needed
• Perform additional duties as assigned

• Must be in good standing with the Minnesota Board of Nursing and have current MN Nursing license
• Must be able to travel throughout the metro area
• Valid Driver’s license
• Experience in assisted living required.
• Detail-oriented and self-motivated.
• Understand, interpret, and utilize data to improve resident care.
• Excellent interpersonal, communication, customer service, and organization skills.
• Ability to manage multiple tasks/projects and deadlines.
• Must be flexible to work in memory care, traditional assisted living and care suites.
