CASA-Trinity is Hiring a Care Manager Near Elmira, NY

This position is responsible for managing an assigned caseload of clients and the delivery of person-centered care coordination under Health Homes.

The schedule would be Monday through Friday, a portion of these hours will be in the clinic and remote.

Essential Job Functions

· Provide direct care management services while maintaining accurate and verifiable documentation of all services rendered.

· Conducts case management assessments as needed.

· Provide a core service for each client monthly as identified in the care plan to ensure the client's goals are addressed.

· Develops, revises, and implements comprehensive service plans in cooperation with participants that include: vocational assessments and description of services, job skills training, employment readiness training and/or educational remediation, life, parenting and social skills trainings, personal, social and community skills training and development, training in community living skills, personal hygiene and personal care skills , and structured daily activities.

· Complete comprehensive and detailed assessments, reviews, crisis plans, and care plans as required by the Department of Health.

· Oversee implementation of plans to the satisfaction of the clients served.

· Review and update plans in consultation with client whenever a change in services requires.

· Arrange for and manage coordination of medical care and other applicable services as needed for clients.

· Provide assistance to clients in accessing community-based supports.

· Provide crisis intervention and follow-up.

· Participate in all mandatory training.

· Operate agency vehicle for mobile services, client transportation etc.

· Assists participants in all obtaining access to all aspects of care, including job skills, employment, education, housing, child care, intervention services, substance abuse education, mentoring, coaching, and spiritual and faith support etc.

· Effectively utilizes agency’s medical records software program(s) to complete required medical record documentation in a timely and comprehensive manner and in compliance with federal regulations including Public Health Law, 42CFR and patient confidentiality policies.

Secondary Functions

· Collaborate and coordinate with other care providers and referral sources to increase understanding of client issues and assets.

· Participates as a contributing member of an interdisciplinary team in coordination and delivery of care.

· Participates in professional supervision and education that supports continuous improvement of care including daily morning case management meetings and clinic case reviews where case manager will present caseload.

· Consistently reviews policies and procedures and stays abreast of current laws, regulations and policies.

· Performs other related work as assigned.

Knowledge, Skills and Abilities

· Introductory knowledge of substance abuse problems and their effects on the individual and family members including human behavior, attitudes, and motivations and their applicability in the treatment/rehabilitation of the substance abuser and/or offender.

· Ability to function autonomously, to make decisions, to problem solve, and to handle emergencies.

· Introductory knowledge of case management practices and techniques.

· Introductory knowledge of resources available to individuals with substance abuse problems.

· Ability to recognize symptoms of substance abuse and elicit information concerning underlying reasons for addiction.

· Ability to effectively manage a caseload, conduct assessments, write assessment summaries, and develop client progress reports.

· Ability to accurately prepare and maintain documentation of services, case management records, and reports utilizing computer databases and software programs including proficiency with spreadsheets.

· Excellent communication skills both written and verbal.

· Ability to establish and maintain effective working relationships with staff, volunteers, and the general public.

Physical Demands

· Use a computer including keyboard and mouse operations; visual and/or auditory capacity.

· The employee may lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds

· Drive a vehicle and sit for longer periods of time in a vehicle while traveling throughout assigned county.

Work Environment

The employee is expected to adhere to strict policies/regulations relative to being in a tobacco and alcohol free workplace.

Qualifications

Must meet one of the educational criteria and one of the experience criteria listed below:

1. Education

a. A Bachelor’s degree with a major or concentration in one of the following fields:

i. Social work

ii. Psychology

iii. Nursing

iv. Rehabilitation

v. Education

vi. Occupational therapy / Physical therapy

vii. Recreation or recreation therapy

viii. Counseling

ix. Community mental illness

x. Child and family studies

xi. Sociology

xii. Speech and hearing

xiii. Other human services field

b. A NYS teacher’s certificate for which a bachelor’s degree is required

c. NYS licensure and registration as a Registered Nurse and a bachelor’s degree

d. A Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses

e. A Credentialed Alcoholism and Substance Abuse Counselor (CASAC)

2. Experience

a. Two years of experience:

i. In providing direct services to people with Serious Mental Illness, developmental disabilities, or substance use disorders;

OR

ii. In linking individuals with Serious Mental Illness, developmental disabilities, or substance use disorders to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing and financial services).

b. A master’s degree in one of the qualifying education fields may be substituted for one year of Experience.

Must possess a valid driver’s license and be insurable. This position is subject to a criminal background clearance.
