Casa Pacifica is Hiring a Care Coordinator Near Camarillo, CA

Wage band range $20.75 - $30.00 per hour; however, starting pay is ultimately determined by the scope of the position, the candidate's relevant experience, and internal equity. Position is eligible for Bilingual pay incentive.

Under the direction of the Clinical Supervisor, is responsible for the systematic coordination / unified care planning between all members of a youth's and family treatment team and partnering agencies involved in their care from admission to discharge. The Care Coordinator carries out the mission of Casa Pacifica as it relates to care coordination and treatment, with the goal of achieving best outcomes for youth and their families.

DUTIES AND RESPONSIBILITIES (illustrated by typical activities):

  • Serves as liaison to parents and partnering agencies. Completes collaborative map and shares with youth's treatment team. Forms solid working relationship with all people involved with youth's care.
  • Upon intake gathers and completes demographic data or each youth on their case load.
  • Creates and maintains a master calendar for each youth. Coordinates and calendars appointments and activities arranging all details in concert with treatment team members.
  • Provides ongoing needed documentation/paperwork (ROI’s, additional consents, etc.)
  • May participate in appointments with youth when clinicians are not available.
  • Works to connect/link youth to community resources. Partners with Clinician to support with Discharge planning. Helps youth connect to resources in the community, including outside recreational activities, substance abuse groups, and assists with independent living activities in conjunction with goals specified on the treatment plan and Individualized Education Plan.
  • Supports Family Day, helping with coordination and outreach.
  • Collects and tracks data for youth outcomes.
  • Supports youth within the cottage and school program.
  • Works collaboratively with School Liaison to track progress of youth in school and maintain a close working relationship with school personnel.
  • Document all services in Avatar including case management, collateral and rehabilitative services the same day they are provided. Coordinate and take minutes for pre-treatment and treatment team and/or Child and Family Team (CFT)meetings and send copies to all team members.
  • Coordinate case paperwork and obtain signatures for client plans and like documents.
  • Coordinate client discharges and discharge planning, ensuring a warm and successful transition.
  • Collaborate with treatment team to coordinate youth visits and engagement with caregivers, family members, and other natural supports.
  • Participate actively in Camino a Casa program meetings helping to advocate for needs of each client.
  • Send incident reports out to team members. Follow up with phone calls to parents and partnering agencies to let them know about incident and help filter any concerns. Notify clinician of concern.
  • Document services and outcomes in quarterly reports.
  • Co-facilitates structured therapeutic groups and process groups with clinician or other Camino a Casa staff.
  • Therapeutic support to children enrolled in the Camino a Casa program including behavioral coaching, problem solving skills, anger management skills, and social skills training.
  • Problem solves as necessary with members of the treatment team and answers questions concerning treatment plan and inter-disciplinary communications.
  • Attends and participates in appropriate staff training programs; attends outside training events and conferences as requested by clinical supervisor.
  • Maintains confidentiality in all matters within the organization with respect to personnel, operations and children.
  • May perform other duties as assigned from time to time.

Position requires staff must be at least 21 years of age. Completed and cleared Livescan, criminal statement, health screening with TB test.

EDUCATION and/or EXPERIENCE:

Must meet at least one of the following requirements prior to employment:

  • Bachelor of Arts or Sciences Degree; degree in social work or related is preferred.
  • Have a valid Child Development Training Permit, or Vocational Training Certificate; or completed 12 semester units of Early Childhood education and have at least 100 hours of experience working with youth.
  • Previously been employed as a staff or volunteer at a group home or short-term residential therapeutic center for 1 year, and/or prior experience working with severely emotionally disturbed children and adolescents.
  • Relevant life experience in the child welfare, mental health or juvenile justice system as a consumer, mentor, or caregiver or other relevant experience as determined by the department.

In Addition, individual must meet one of the following billing requirements:

  • A minimum two (2) years of full-time experience providing direct care experience in a mental health setting and/or support services to mental health clients and their families

OR

2. Possesses a bachelor’s degree in a mental health related field (Social-Behavioral Sciences including psychology, social work, or sociology) AND A minimum of six (6) months of full time experience providing direct care experience in a mental health setting and/or support services to mental health clients and their families.

OTHER SKILLS AND ABILITIES: Must have good, understanding of child and adolescent development, behavior modification, teamwork, and group process. Must have aptitude to deal with severely emotionally disturbed adolescents in a variety of settings. Ability to use various computer software (Microsoft Office 365). Should have strong organizational, communication, and leadership skills; strong human-relations/interpersonal skills; and impeccable manners. Bilingual (Spanish) is strongly preferred.

Transportation of youth/families: Valid California Driver's License with excellent driving record. Use of personal car may be required, mileage reimbursement. Must have proof of insurance. Must maintain a positive driving record; reports tickets and driving points to Program Manager as they occur. Abides by Casa Pacifica travel policy. Must be agency approved driver.
