Wage band range $21.00 - $34.00 per hour; however, starting pay is ultimately determined by the scope of the position, the candidate's relevant experience, and internal equity. 

Under the guidance and supervision of the ISFC/FFA Supervising Social Worker/Program Coordinator, the case manager utilizes a clinical skill set to begin assessment to determine mental health needs and develop treatment plans. The Case Manager provides therapeutic interventions, collateral and some case management services to the ISFC youth, resource parents and/or family of origin. The Case Manager/ Clinician should have a Master’s Degree and preferably be a licensed LCSW, or be licensed eligible ASW.

DUTIES AND RESPONSIBILITIES (illustrated by typical activities):

  • Provides case management services to children and families
  • Provides therapeutic interventions including Social, behavioral, emotional, and communication skills for children and parent/guardian as needed at home, school, or other community based settings in accordance with the Needs and Services Plan and treatment plan.
  • Participates as a member of the child's treatment team and works closely with all treatment team members regarding matters pertaining to all one-to-one individualized behavioral interventions as outlined
  • Maintains regular contact with, and support for family and child/non-minor dependent and monitors psychological, mental, emotional, physical, and environmental well-being through phone contacts, email and home visits.
  • Implements parent/guardian coaching/training, and assists other significant persons involved on the treatment team in coaching and transferring strategies and skills necessary to help foster children/non-minor dependents and foster parents meet their established goals outlined in the Needs and Services Plan.
  • Lead a quarterly Progress Review to review the individual’s progress and assist the client and team in making revisions to his/her Needs and Assessment Plan as needed.
  • Remain available to clients for family meetings including visitation with birth/natural parents.
  • Attend strategic meetings when there is an urgent need to modify the existing plan due to conflicts and problems.
  • Participates in development of assessment, service delivery, discharge, and aftercare planning.
  • Maintains an active client caseload of no more than 15 cases.
  • Provides crisis intervention on a 24-hour on call basis to support foster families during times of escalated high-risk behaviors.
  • Attends weekly Case Supervision and monthly individual supervision with assigned Clinical Supervisor.
  • Delivers support to foster families in compliance with Community Care Licensing Title 22 licensing/regulatory standards.
  • Facilitates a quarterly Progress Review with the individual foster child/non-minor and other team members to determine his/her progress toward goals outlined in the individual Needs and Services Plan and to assist the child/non-minor dependent in making revisions to his/her individual Needs and Services plan when necessary.
  • Adheres to the legal and ethical requirements including but not limited to mandated reporting laws, as outlined in the mental health profession code of ethics and as deemed necessary by the agency.
  • Maintains organized progress notes for each client contact, submitting notes in a timely manner by the 24 hour deadline.
  • Facilitates decisions that are collaborative and include all treatment team members Keeps FFA Clinical Supervisor, Ventura County Placement Social Worker treatment, and natural parents and all necessary others apprised of day to day progress of foster child/non-minor dependent.
  • Will participate in foster parent trainings.
  • Maintains effective lines of communication with Clinical Team and Program Management with regard to program needs.
  • Attends and participates in appropriate staff trainings; attends outside training events and conferences as requested.
  • Maintains confidentiality and HIPPA regulations in all matters within the organization with respect to personnel, operations and children.
  • Provides resource information, linkages and referrals to additional community organizations for continued treatment and support.
  • May perform any other additional duties as assigned, including but not limited to case coverage for other Social Workers when necessary and if case load permits.


EDUCATION and/or EXPERIENCE: The Foster Family Agency Social Worker Case Manager shall possess a Master’s degree in the following: Social Work. Social Worker Case Managers shall have at least four years related experience and/or training; or equivalent combination of education and experience. A minimum one-year experience in working with clients with special needs or developmental disabilities in a Foster Family Agency or Community Based Program is preferred.

CREDENTIALS: Casa Pacifica SAFE certification, Community First Aid and Cardiopulmonary Resuscitation (CPR) certification, Mandated reporting.

OTHER SKILLS AND ABILITIES: Must have good working understanding of child and adolescent development, behavior management, and family dynamics including working with cultural and diverse populations. Must have aptitude to deal with severely emotionally disturbed children/adolescents in a variety of settings. Ability to use various computer software (Microsoft, Office, Windows, Word, Excel, Access). Bilingual in Spanish, preferred. Strong organizational, communication and leadership skills. Strong human relations skills and excellent interpersonal skills. Flexibility, time management skills and self-motivation required.

Valid California Driver's License with excellent driving record. Use of personal car required; mileage reimbursement. Must have proof of insurance. Must maintain a positive driving record; reports tickets and driving points to TBS Program Manager as they occur. Abides by Casa Pacifica travel policy.
