Wage band range $75,000.00 - $95,000.00 annually. However, starting pay is ultimately determined by the scope of the position, the candidate's relevant experience, and internal equity. Position is eligible for Bilingual pay incentive. 

Under the direction of the Chief Operating Officer, this position will provide mental health services to young adults in our Transition Youth Services Program (Stepping-Stones) as well as youth from the community. In addition, they will have a supervisory role in our Enhanced Care Management program, ensuring quality of services. As a Licensed therapist, must maintain a high level of clinical and organizational skills and carry out the mission of Casa Pacifica. Other duties and responsibilities may be assigned to support the youth and young adult population in the respective departments.

CREDENTIALS: LPCC, LCSW, or LMFT with California license.

DUTIES AND RESPONSIBILITIES (illustrated by typical activities):

Outpatient Program

  • Provides mental services to youth in Stepping-Stones program and community.
  • Will need to go through credentialling process with various insurance companies.
  • Completes clinical documentation in agency electronic health records.
  • Maintains compliance with all local, County, State, and Federal government regulations to Including Title 22, Division 6, and prepares reports required by regulatory agencies.
  • Maintains confidentiality in all matters within the organization with respect to personnel, operations, and clients.
  • May perform other duties as assigned from time to time.

Enhanced Care Management

  • Under the direction of the ECM Services Manager ensures the quality of services provided by ECM Lead Care Managers addresses the clinical and non-clinical circumstances of high-need clients enrolled in Medi-Cal managed care.
  • Provides direct supervision for ECM Lead Care Managers and supports ECM Services Unit activities related to staff training, assessment, quality assurance and improvement, and evaluation of the services.
  • Provides clinical support and assistance to ECM Lead Care Managers through routine rounding/interdisciplinary care team meetings.
  • Ensures that ECM strategies and services are whole person centered, linguistically, and culturally appropriate.
  • Monitors quality improvement measures for coordination of care through documentation review, clinical consultations, peer review, and other related activities to ensure quality of services.
  • Oversees charting and documentation standards adhering to ECM contracts with Managed Care Plans (MCPs).
  • ECM services provide a whole-person approach to children, youth and families experiencing complex medical and psychosocial challenges to minimize the likelihood of preventable hospital and emergency department admissions. Services include outreach and engagement, providing person-centered care coordination, developing care plans, and providing linkages to medical, psychiatric, social, educational, and other services as needed.


EDUCATION and/or EXPERIENCE: Licensed Masters supervisor professional experience with three to five years’ experience working with severely emotionally disturbed youth. 0-3 post licensed master level.

CREDENTIALS: LPCC, LCSW, or LMFT with California license.

Position requires staff must be at least 21 years of age. Completed and cleared Livescan, criminal statement, health screening with TB test.

Minimum Requirements / Qualifications (All applicants MUST meet the minimum qualifications):

  • Minimum of two (2) years of experience in care coordination and case management for focal population such as children and adults experiencing Mental Illness (SMI) and/ or Substance Use Disorder (SUD) or children involved in or with a history of involvement in child welfare.
  • Assesses the clinical needs of the youth, develops and revises treatment plans, makes referrals to needed resources.
  • Provides clinical services such as individual, collateral, rehabilitation, parenting skills training, and case management as appropriate to the individual needs of the youth.
  • Evaluates the changing mental health needs of the youth and adjusts accordingly.
  • Utilizes evidenced based practices that are appropriate to meet the clinical needs of the youth and family.
  • Acts as liaison with placement agencies and community services and works closely with mental health and child protective services, as well as law enforcement agencies, schools, employers, health and welfare, and recreation agencies to provide coordinated care.
  • Confers with teaching staff regarding the behavioral or learning problems of youth and recommends methods of modifying inappropriate behavior and encouraging learning experiences.
  • Confers with parents regarding school and facility activities, policies, and procedures as needed and appropriate.
  • Ensures clinical documentation and quality assurance meets both agency and payor standards, ensuring audit-ready charts and continuous quality improvement.
  • A valid Class C California Driver’s License, access to personal automobile, and proof of auto insurance.
