About the role

  • The Tu Bienestar project is aimed at increasing engagement in the care of
    Latinxs with substance use and co-occurring mental disorders who are at risk for or living with HIV and other infections (including VH). The Outreach Worker will conduct outreach and educational activities to engage clients in Casa Esperanza’s services, facilitate access to recovery-oriented substance-use-disorder (SUD) treatment, resources, and recovery groups; provide referrals for HIV/STI/Hep A, B, C individual and group intervention; and provide other psychosocial support and mentoring for clients living with
    HIV/AIDS.
  • As part of the Casa Esperanza, Inc. team, you will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach to ensure the coordination of all client services, both across Casa programs and with external providers. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessary follow-up and assignments; and ensure-wrap around support services are in place to maximize treatment effectiveness and clinical outcomes.
    What you'll do:
    Responsibilities:
  • Work with Project Manager to implement in-reach and out-reach strategies to identify and recruit clients for Outpatient HIV support services, including street outreach and presentations to community organizations. Collaborate with the Marketing team on the development of collateral materials and social media presence to create awareness and connection to resources.
  • Develop and maintain partnerships with other HIV-focused organizations, clinical providers, and other Latinx-serving organizations.
  • Provide ongoing education regarding substance use, mental health, HIV, and other health and wellness information to clients and their family members.
  • Assist with off-site activities related to Health and Wellness programming
  • Conduct community visits and follow-ups to re-connect with patients who are not engaging in care
  • Provide information regarding PrEP, nPEP, and Twinrix; distribute safety supplies; and provide referral to recovery support and treatment services
  • Connect the population to treatment, recovery support services, and HIV services at Casa Esperanza and at other agencies across Greater Boston.
  • Provide transportation assistance to and from treatment and support services
  • Work closely with MDT to engage and retain clients in the care
  • Represent the agency by participating in community events and delivering educational presentations as needed.
  • Willingly share personal recovery experiences, if applicable, demonstrate and model effective coping techniques and self-help strategies.
    Administrative:
  • Document all client encounters and other data as required and maintain up-to-date client records.
  • Document all outreach encounters with organizational affiliates in Casa database
  • Attend bi-weekly supervision.
  • Write and submit all reporting as required by Program funding sources in a timely manner
  • Attend all relevant case conferences and staff meetings, including MDT.
  • Other duties as assigned
    Qualifications:
  • Bilingual Spanish/English required.
  • Knowledge of HIV, recovery process and resources
  • Experience doing outreach to individuals at risk for HIV, addiction, mental illness, trauma and violence.
  • Able to work weekends and evenings, as needed OR Must be available to work weekends and two evenings a week.
  • Familiar with social, health and cultural issues specific to the Latino population
  • Ability to work as a team member & independently
  • Understanding of harm reduction techniques
  • Strong verbal and written communication skills
  • Strong interpersonal skills and crisis management skills
  • Knowledge of HIV/AIDS health screenings and service planning and education
  • Minimum High School Diploma required;
  • Computer skills, minimum Microsoft Word.
  • Valid Massachusetts Driver’s License

Schedule
Full Time (40 Hours)
Mon to Fri - Availability 8am to 8pm
Some weekends required per program needs

While performing the duties of this job the employee is regularly required to walk, stoop, kneel, crouch or crawl and talk or hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include distance vision, peripheral vision, and depth perception.
