Assists Case Management Director with unit administrative tasks; performing standard and advanced secretarial duties. Coordinate communication independently based on knowledge of policies and procedures, or from verbal direction. Provides support for RN Case Managers/Social Workers and interfacing with patients, families, and/or stakeholders to ensure timely transition of patients through the continuum of care to help provide quality transitions, prevent readmissions, and assist with non-licensed Case Management tasks. Handles matters of a routine nature to conserve supervisor’s time. Responsible for completing projects in a timely and efficient manner at supervisor’s request. Handle issues that may be extremely sensitive and confidential in nature. Perform Utilization Management activities that include regular audits/reports, and data collection and analysis for process improvement projects. At least Five (5) years of progressively responsible, complex secretarial/database experience or college level class work in business administration or related fields; or an equivalent combination of training and experience.
