Carle Health is Hiring a Weekend Medical Social Worker Near Bloomington, IL

Position Summary:
Sign on bonus - $2,500.00 for experienced or $1,000.00 for non experienced - External only
Provides social work services to patients and families within hospital setting in order to meet identified psychosocial needs, as well as to enhance problem-solving and coping capacities and to facilitate timely discharge by arranging post-discharge services. The social worker provides psychosocial assessments, supportive counseling, crisis intervention, community resource connection, financial resources and works collaboratively with all other members of the healthcare team, ensuring clear communication and care planning to all patients.

EDUCATIONAL REQUIREMENTS
Master's Degree in Social Work Bachelor's Degree in Social Work
CERTIFICATION & LICENSURE REQUIREMENTS
Licensed Clinical Social Worker (LCSW) Illinois upon hire or Licensed Social Worker (LSW) Illinois upon hire.
EXPERIENCE REQUIREMENTS

ADDITIONAL REQUIREMENTS

  • Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients
  • Completes all requireduired Continuing Education to maintain licensure and increase knowledge within area of practice speciality
  • Completes and utilizes all annual safety and competency training.
  • Maintains Up to date knowledge of community resources, legislation and regulations impacting health care delivery and providing education to patients and families as appropriate.
SKILLS AND KNOWLEDGE
Ability to coordinate multiple projects concurrently and establish priorities. Possesses strong planning and organizational skills. Self-motivated. Requires flexibility in hours and job duties. Ability to work effectively with diverse individuals and groups. Ability to identify at risk populations and supply appropriate SW interventions. Maintenance of professional standards and responsibilities for his/her own professional practice according to accreditation, hospital, system, state and NASW Standards and Code of Ethics

Essential Functions:

  • Assess patients who are uninsured/underinsured for IL Medicaid eligibility and/or Carle Financial Assistance Program eligibility. If so, provide direction or assistance with completing the application(s).
  • Complete psychosocial assessments with patients to identify expressed, implied or anticipated needs.
  • Complete mandated reports of suspected abuse and/or neglect of minors, disabled adults, and seniors. Cooperate and collaborate with the investigating bodies for safe plans.
  • Find solutions to offer patients so that they can obtain needed prescription medications, e.g coupons, copay cards, pharmaceutical assistance programs, Social Security Extra Help, 340B, One Time Rx Assistance, etc.
  • Find, and if possible, arrange transportation for patients to get to and from their medical appointments and/or home from the Emergency Department/Hospital.
  • Educate about and connect patients with community resources to help meet their needs, e.g. non skilled help at home, meals/food, housing resources, charity assistance programs, support groups, etc.
  • Provide empathic listening and emotional support to patients either in person or over the phone.
