Job Description: Advantage Case Manager

Position Overview:

We are seeking a dedicated and compassionate Advantage Case Manager to join our team and provide exceptional care coordination and support to clients. As an Advantage Case Manager, you will be responsible for assessing clients' physical and mental wellness, preferences, and needs, in order to develop personalized care plans. You will collaborate with clients, their families, support networks, and healthcare professionals to implement effective care strategies. Your role will involve providing counseling, intervention, and monitoring progress to ensure optimal outcomes.

Key Responsibilities:

· Conduct thorough assessments of clients' physical and mental health, preferences, abilities, and needs, with a focus on creating tailored care plans.

· Collaborate with clients, their families, friends, and healthcare professionals to establish and implement comprehensive care plans that address individual requirements.

· Act as a compassionate listener and provide counseling or intervention to address clients' concerns, ensuring their emotional well-being.

· Document clients' progress accurately, maintaining detailed records of referrals, home visits, and significant interactions for future reference.

· Regularly evaluate clients' progress by tracking outcomes, adjusting care plans as necessary to enhance results, and facilitating continuous improvement.

· Initiate follow-up with clients after discharge to assess their satisfaction with services received and confirm their sustained physical and mental well-being.

· Perform comprehensive assessments that encompass various dimensions of clients' lives to create a holistic view of their needs.

· Develop and implement effective service plans that align with clients' objectives and resource availability.

· Oversee the implementation of service plans, coordinating with service providers, healthcare teams, and community resources.

· Monitor the performance of service providers and members, ensuring that care plans are executed in accordance with established guidelines.

· Conduct regular reassessments of clients' situations to identify changes, adapt care plans, and guarantee ongoing relevance and effectiveness.

Qualifications:

· Bachelor's degree in social work, nursing, psychology, or a related field (Master's degree preferred).

· Previous experience as a Licensed Practical Nurse (LPN), Home Health Aide (HHA), Caregiver, Certified Nursing Assistant / Nursing Assistant (CNA) preferred.

· Proven experience in case management, care coordination, or a related role.
Strong understanding of physical and mental health assessments, care planning, and intervention strategies.

· Excellent communication skills, both written and verbal, with the ability to empathetically engage with clients and stakeholders.

· Proficiency in maintaining accurate records and documentation.

· Ability to work collaboratively in interdisciplinary teams and build effective relationships with healthcare professionals, clients, and families.

· Exceptional organizational skills and attention to detail.
Knowledge of relevant regulations, policies, and ethical considerations in healthcare and case management.

· Proficiency in using electronic health records (EHR) systems and other relevant software.
