Arrive to shift on time. Reports to Director of Nursing.

RESPONSIBILITIES:

To provide nursingcareto residents as prescribed bythe physicianand in accordancewith standards ofnursingpractices, regulationsand directed bythe Director ofNursing.To lead or direct licensedand non-professional staff in thedeliveryof direct Residentcare and support functions.

QUALIFICATIONS:

  • Must havea currentLVN/RN licenses.
  • Abilityto understand, remember and carryout oral or written instructions in English.
  • Abilitytocomplete assignments in atimelymanner.
  • Abilityto be flexible and positivewith intermittent interruptions.
  • Must have at least one(1)year of experiencein eitheran acute medical/nursingcenter or alongterm carecenter.

ESSENTIAL FUNCTIONS:

  • Musthaveworkingknowledgeofthebasicprinciplesofinfectioncontrolandwound care.
  • Must have demonstrated competence in providing wound care that is sterile and/or aseptic.
  • Musthaveknowledgeandunderstandingofthespecialneedsofthegeriatricclientand the geriatricclient’s skin.
  • Mustbefreeofanycommunicablediseaseandmustmaintainhandswithclean,non- jaggednailswhichareofappropriatelengthtoassure residentsarecaredforinariskfree manner.
  • Provides wound care (bandages, cleaning, applying medication) and assessing for appropriateness of bandageintegrityduringtime between dressingchanges.
  • Assuringavailabilityoftreatmentsuppliesandtopicalmedications(asprescribedbythe physician)andprocuringsuppliestoassureadequateinventoryorTreatmentCartis readilyavailable at all times.
  • Providing gastrostomytube feeding care, includingneeded bandagechanges.
  • Providingcolostomyandurostomycare and dressings as directed bythe physician.
  • Provideweeklydocumentation.
  • Complies with facilityguidelines fordocumentation and physicians orders.
  • Relates well to patients, families andotherteam members.
  • Interacts well with therapists andimplements individual programs as developed.
  • Maintains equipment andsupplies fortreatment.
  • Maintainsconsistentpatientcarescheduleandisefficientinuseoftime.Coordinates restorativetreatments with otherfacilitydepartments.
  • Ableto perform duties with aminimum of supervision.
  • Identifies potential problems/issues and reports tosupervisorin a timelymanner.
  • Maintains aprofessionalapproachwith confidentiality.
  • Is supportiveof facilitygoals and objectives.
  • Isable to useAccuNursesystem to convey/receive resident information.
  • Cooperates and maintains good rapport with nursing staff, medical staff, other departments, and visitors.
  • Obtainsanddocumentsnursinghistoryfromthepatientand/orsignificantotherthatis comprehensive of present condition and prior problems.
  • Performsbasicassessmentofthepatient,indicatingadequateknowledgeofanatomy, physiology, and pathology.
  • Records vital signs, noteschanges, andpursues more-specific investigationas needed.
  • Assesses forand notifiesphysicianand other appropriate parties of changes in condition.
  • Usesassessmentinformationtodevelopacareplanbeforetheendofdutytimethat communicatesenoughinformationforincomingpersonneltoadequatelycareforthe patient.
  • Uses careplans asabasis forprovidingsafe and therapeuticcareto patients.
  • Consistentlyfollowsestablishedstandards,policies,andproceduresinprovidingnursing care.
  • Acceptsaccountabilityforclinicalcareofassignedpatients,includingsupervisionof nonprofessional personnel.
  • Communicateswiththepatientandhisorherfamilyandothermembersofthehealth careteam in carryingoutthe goals of the careplan.
  • Performscompetentlythosetechnicalskillsconsideredbasictothetypeofcaregivenon that unit.
  • Responds appropriatelyin urgent and/or emergencysituations.
