Description

Position Summary

1. The Program Assistant will provide personal care for participants in the program, lead activities, assist the Adult Day Services Supervisor with activities, and help serve meals and snacks. Program Assistant must be able to carry out the daily routine at the program site without constant supervision and is under the supervision of the Adult Day Services Supervisor and the Director of Programs.
2. The Driver’s primary responsibility is to transport clients to and from the Adult Day Service Program. This includes assisting passengers with disabilities, with wheelchair loading and securing the wheelchair with the proper restraint procedure. Schedule varies depending on the needs of the Center.

Duties and Responsibilities

1. Sets up room for daily program.
2. Maintains neatness and order in rooms.
3. Assist clients in arriving and leaving program site. Including removal of coats, assisting with walkers and wheelchairs.
4. Organizes, prepares, and helps distribute lunch and snacks.
5. Leads daily activities on a regular basis, including exercise, AM and PM groups.
6. Assists Activity Coordinator with special events and parties.
7. Takes clients for walks, and outings. 
8. Keeps Activity Coordinator informed on needed supplies.
9. Socializes with clients.
10. Reports unusual behavior of clients (disposition, wandering) to Activity Coordinator or Nurse
11. Toilets Clients per client’s individual care plan.
12. May open and close the premises.
13. Informs Supervisor of all schedule changes within an appropriate amount of time, i.e., calls in sick as soon as possible.
14. Communicates in an appropriate manner with clients, caregivers, and volunteers.
15. Cleans up after activities, returning items to the proper location in supplies closet.
16. Daily documentation of activites, behaviors and toileting schedule.
17. Monitoring of clients safety and whereabouts at all time.
18. Operating a Wheelchair equipped van or Passenger Van
19. Assisting clients on and off of the van.
20. Responsible for daily safety check on the vehicle.
21. Responsible for refueling and cleaning the vehicle.
22. Operating the van in the safest manner.
23. Informing the Supervisor of any maintenance problems.
24. Maintain cooperative work relationships.
25. Ability to work outdoors sometimes in inclement weather.
26. Ability to understand and maintain time points and schedules.
27. Ability to read a map.
28. Able to work with a minimum of supervision.
29. Ability to exercise sound judgment in emergencies.
30. Knowledge of the geography of the city and names and locations of streets.
31. Securing wheelchairs in standard to ADA
32. Assisting confused clients to and from the vehicle.
33. Insuring all clients are properly restrained.
34. Filling out Credit Card Receipts for charged fuel.
35. Ability to operate a vehicle safely while observing passenger activities in the van and at all van stops.
36. Ability to successfully complete a certified first aid and CPR class and to correctly apply training to clients as required.
37. Other duties as assigned.

Requirements

Qualifications

1. Have a high school diploma and demonstrable aptitude in working with individuals affected by Alzheimer's disease and related dementias.
2. Valid chauffeur’s license and proof of current automobile insurance if driving agency vehicles.
3. Reliable transportation to work.
4. Must be responsible and work well with others.
5. Experienced in working with the elderly, handicapped, or dementia patients is preferred
6. TB Test upon hire and retest every three years.
7. Undergo a LIEN check with no felony convictions and no abuse/neglect convictions.
