Overview:
We are a medical group practice of outstanding, hand-picked clinicians making more than 1,000 home visits a year in private residences, retirement communities, apartment buildings, and independent and assisted facilities. We are driven to bring care to private residences, retirement communities, apartment buildings and independent and assisted facilities. Our passion is caring for frail, elderly complex patients, – particularly the highest-needs, in their home wherever they reside.

We are looking for a compassionate Nurse Practitioner to join our team and provide primary care services to our patients. The ideal candidate will have a passion for patient care, excellent communication skills, and the ability to work independently. The Nurse Practitioner will be responsible for providing advanced primary and/or specialty care to patients primarily in patient homes. Duties will include, but are not limited to, disease management, performing physical exams, recording patient medical histories, and creating patient care plans that are aligned with patient’s goals.

This position involves daily travel.
Responsibilities:
  • Provide extensive primary care to patients, with the goal of helping the patient to age in place.
  • Provide Advance Care Planning discussion to illicit patient’s state goals of “What Matters Most”.
  • Assist the practice and participates with the Practice Administrator and Home-Based Primary Care Physician in meeting safety, performance, and quality measures for a Home-Based Primary Care Practice.
  • Work collaboratively with other lines and HBPC interdisciplinary team to achieve patient and model goals.
  • Triage incoming messages, completed within the same day as the message was received.
  • Perform physical exams and patient observations.
  • Record patient medical histories and symptoms.
  • Order and analyze diagnostic tests.
  • Appropriately prescribing medicines within the scope of prescriptive authority including schedule II-controlled substances.
  • Use of most appropriate and safest approaches to pain management.
  • Diagnose health issues.
  • Detect changes in a patient’s health, change in condition, and change treatment plans as necessary.
  • Create patient care plans and contribute to existing plans.
  • Involve community resources as needed.
  • Consult with healthcare professionals.
  • Train patients and families on managing illnesses or injuries and disease prevention.
  • APN authorization in state where employed
Qualifications:
  • Master’s degree from an accredited nursing program
  • RN licensure in the state where employed or a compact party state and be board certified by either ANCC or AANP
  • CPR Certified
  • Must be able to lift a minimum of 50 pounds.
  • Valid driver’s license
Physical Requirements:
  • Ability to interact with patients for physical assessments, provide assistance, without causing harm or aggravating injuries to the patient.
Can successfully perform the essential job functions consistent with the ADA, FMLA and other Federal, State and Local standards, including meeting qualitative and/or quantitative productivity standards.
