CareOne Home Health is seeking a Speech Language Pathologist to add to our growing team of highly-regarded health professionals. The Speech Language Pathologist is responsible for speech therapy services for patients in San Joaquin county and surrounding communities.

Requirements:

• Possesses a master’s or doctoral degree in speech-language pathology (SLP), and licensed speech language pathologist in the state of California.
• Minimum 1 year(s) experience as a working SLP
• 1 year home health experience preferred
• Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
• Experience with Kinnser (WellSky) EMR preferred
• Demonstrates good verbal and written communication, and organization skills
• Possesses and maintains current CPR certification

Primary Responsibilities:

• Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function. Ensuring coordination of care with other team members and marketing team
• Identifies functional speech defects and establishes a plan of care to improve patient’s function.
• Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
• May supervise home health aide in procedures directly related to the speech–language therapy plan of care
• Performs other duties as required to facilitate the delivery of speech language pathology services.
