Caregivers Home Health Services is Hiring a Hospice Registered Nurse Near Alexandria, VA
- Coordinates hospice care by workingwith other team members as well as with the patients/family members to ensureneeds are met.
- Evaluate the effectiveness ofhospice services to the patient and family on an ongoing basis.
- Perform evaluations for hospice carealong with informational visits as needed.
- Utilize the nursing process toprovide professional nursing care.
- Consult with the attendingphysician and Hospice Medical Director concerning changes in patients’condition requiring order changes and/or changes in the plan of care.
- Coordinate patient services withother members of the IDG as well as contracted services, when needed.
- Submit complete, accurate andrelevant clinical notes in a timely manner.
- Submit patient visit scheduleweekly.
- Participate in IDG and caseconferences.
- Provide guidance and supervision tothe LPN and supervises the LPN a minimum of every 30 days. Cooperate with otheragencies providing nursing or related services to provide continuity of careand to implement a comprehensive care plan.
- Participate in staff developmentmeetings.
- Prepare the care plan for theHospice Aide/Home Attendant.
Requirements
- Must be a graduate from an accredited School of Nursing.
- Must be licensed in the State of Virginia as a RegisteredNurse.
- A minimum of two (2) years of hospice/home care experience;at least one (1) year of supervisory experience.
- Must have excellent assessment skills and problem-solvingskills.
- Should be skillful in organization and in the principles oftime management and have knowledge of case management processes.
- Must be a licensed driver with an insured automobile in goodworking order.
- The ability to communicate well, both verbally and inwriting.
