Caregiver Grove is Hiring a Program Manager - Development Disability Services Near Toledo, OH

As the Developmental Disability Services Program Manager you will lead and support our team of Direct Support Professionals (DSPs) in delivering high-quality services to our clients. This hands-on leadership role requires ensuring that our policies, procedures, and service delivery align with our mission and comply with contract requirements, as well as state and federal regulations. You will oversee the overall health and safety of the individuals we serve while optimizing the utilization of available resources for efficient and effective program management. This role involves filling in where necessary to support our clients.

Essential Job Functions:

Staff/Team:

  • Mentor and guide DSPs in providing exceptional care and support to our clients, ensuring adherence to Individual Service Plan (ISP) contract requirements and agency policies.
  • Collaborate closely with Quality Assurance to maintain and improve service quality, establishing robust systems for quality assurance.
  • Foster positive communication among all staff members and locations to enhance teamwork and cohesion.
  • Conduct performance appraisals and meetings for direct reports, providing constructive feedback and support.
  • Ensure comprehensive training for staff members in accordance with agency guidelines and Ohio Administrative Code (OAC) requirements.
  • Coordinate and lead regular Site Team Meetings to facilitate collaboration and information sharing.

Individual:

  • Review individual-specific ISPs and other relevant services to ensure contractual compliance and alignment with individual needs.
  • Maintain accurate and comprehensive records and documentation, ensuring compliance with program requirements and agency policies, and timely submission of all required reports (e.g., HPCs, QAs, LPS).
  • Address and resolve complaints, grievances, and appeals related to individual services, escalating issues as necessary and ensuring effective communication.
  • Facilitate communication and care coordination among individuals, families, and support teams to promote holistic care and support.
  • Assist with DSP services and fill in where necessary, demonstrating hands-on leadership and support.

QUALIFICATIONS:

  1. Bachelors’ level degree in a related field.
  2. 3 years of experience in the field of developmental disabilities.
  3. Must be certified within 90 days of hire on First Aid, CPR, and De-escalation Training.
  4. Must maintain a current and valid Ohio or Michigan driver’s license, a good driving record, adequate insurance and be willing to use personal care to perform job duties.
  5. Strong organizational, time management, multi-tasking, and problem-solving skills.
  6. Impeccable integrity, judgement, and professionalism.
  7. Must be analytical and solutions-oriented.
  8. Ability to work well in a high-growth, fast-paced environment.
  9. Comprehensive working knowledge of computer systems.

PHYSICAL REQUIREMENTS:

  1. Standing: Maintaining an upright position on the feet, especially for prolonged periods of time
  2. Pushing: Applying pressure and exerting force against an object for the purpose of moving it forward or away from the source of the force – push or pull force up to 35 lbs.
  3. Lifting: Raising an object from a lower to a higher position, or moving an object laterally, from position to position-Required to lift up to 50 lbs of force independently. May be required to lift over 50 lbs with assistance of another person or a mechanical lift.
  4. Stooping: Bending the body forward and down from the waist, hips and knees, sometimes for a prolonged length of time.
  5. Reaching: Extending hand(s) and arm(s) in any direction away from the body
  6. Balancing: Maintaining body equilibrium to prevent falling when walking, standing or crouching on narrow, slippery or moving surfaces
  7. Ability to safely operate a personal motor vehicle on a weekly basis and comply with the Company's Vehicle Safety Program.
