Summary:

The Float Nurse will serve as primary staff member responsible for supporting the clinical hospice team between the hours of 4:30 PM – 8 PM in various roles throughout the CareFirstNY services areas: Steuben, Chemung, and Schuyler counties. Offers flexibility with schedule! Responsibilities include:

• Being part of coverage for caseloads;
• Making frequency visits based on patient/family needs;
• Personal care visits;
• Completing admission as needed.

Duties:

• Share in on-call rotation providing 24-hour, 7-days-a-week coverage to patients and families
• Providing informational visits as assigned
• Provide coverage for Case Managers by completing assessments of patient/family needs, development of care plans, communication/education in a manner to advocate and empower, completing nursing in a fiscally responsible manner
• Participate in interdisciplinary group meetings to update patient/family plan of care as needed
• Carry a patient caseload as assigned by Team Managers per agency need
• Participate in quality assurance program and perform chart audits as assigned
• Consult with supervisors and/or other interdisciplinary team members before acting on changes in patient’s health status, community referral potential, family stresses, and unanticipated nursing or medical needs
• Work with and collaborate with all members of the interdisciplinary team as indicated
• Maintain timely documentation
• Provide supervision and evaluation of hospice and contracted homemakers, home health aides, and LPN’s

Qualified candidates will possess a valid and current NYS Professional Registered Nurse’s license and driver’s license with minimum of 2 years’ experience in home care and/or acute care setting within the last 10 years. Must also possess physical assessment skills, strong communication skills, and be skilled in the use of nursing processes.
