Transitional Care Program Overview: New multi-disciplinary program specifically created to help patients with spinal cord and brain injuries to achieve a higher level of independent functioning in daily living skills through skilled nursing care, rehabilitative services, case management, and psychological services.

Job Responsibilities: 

Clinical

  • Complete new patient evaluations, establish customized treatment plans and patient goals and participate in the implementation of patient care programs
  • Deliver high-quality patient care for multiple rehabilitation diagnoses, primarily brain injury and spinal cord injury
  • Provide a positive patient experience through patient engagement and progressive treatment
  • Promote evidence-based patient care

Administrative 

  • Understand and support the financial goals of the organization and the Transitional Living Program
  • Develop and achieve budgeted metrics for areas of responsibility
  • Maintain staff engagement and development
  • Manage clinical and non-clinical staff to ensure appropriate staffing ratios in order to provide continuity of care for our patients and to meet regulatory requirements
  • Perform employee performance evaluations to support their career development
  • Create a positive patient experience resulting in high outcomes and satisfaction
  • Market the program and services offered, acting as a liaison with all referral sources
  • Assist in creating and developing marketing materials and community activities related to Transitional Living Program 
  • Ensure departmental/program quality control processes are functional and reported adequately and timely
  • Communicate clinic’s operational status of key metrics to all key stakeholders
  • Adhere to all compliance and regulatory standards pertaining to patient treatment and delivery of care in the skilled nursing environment

Job Qualifications:

  • Bachelor’s degree in a therapy field (PT, OT, SLP) required 
  • Licensed therapy professional with a minimum of two (2) years of neurological clinical rehabilitation experience required
  • Specific expertise working with brain and spinal cord injury patients preferred
  • Demonstrated leadership abilities with a high degree of integrity, accountability, and collaboration
  • Knowledge of and expertise in workers compensation preferred 
  • Strong interpersonal and customer service skills required to interact with internal and external constituents
  • Strong analytical skills with ability to research, collect, and compile data with accuracy and precision
  • Must be a proactive problem solver and team player who is detail oriented and flexible; a self-starter requiring minimal supervision with the ability to partner and support requests from multiple sources
  • Outstanding verbal and written communication skills required
